Licensed Vocational Nurse Skilled Nursing Facility

Overview: We are seeking a compassionate and dedicated Licensed Vocational Nurse to join our team at [Skilled Nursing Facility Name]. The LVN will play a pivotal role in providing exceptional care to our residents, ensuring their well-being and comfort within our skilled nursing environment.
Responsibilities:
1. Patient Care:
? Administer nursing care to residents based on individualized care plans and physician orders.
? Perform and document routine assessments, monitor vital signs, and evaluate resident health status regularly.
2. Medication Administration and Treatment:
? Administer medications and treatments as prescribed, adhering strictly to medication administration protocols.
? Assist in the development and implementation of care plans in collaboration with the healthcare team.
3. Wound Care and Monitoring:
? Provide specialized wound care as required, ensuring proper documentation and follow-up.
? Monitor and assess changes in resident conditions, promptly reporting any concerns to the registered nurse or physician.
4. Patient and Family Education:
? Educate residents and their families about their conditions, treatment plans, and self-care strategies.
? Offer emotional support and guidance to residents and their families throughout their stay at the facility.
5. Documentation and Compliance:
? Maintain accurate and up-to-date medical records and documentation in compliance with facility policies and regulatory standards.
? Participate in quality assurance activities and assist in ensuring compliance with state and federal regulations.
Qualifications:
? Active LVN license in Texas.
? Previous experience in a skilled nursing facility or long-term care setting is preferred.
? Proficient in nursing assessments, medication administration, and wound care.
? Strong communication and interpersonal skills.
? Ability to work collaboratively in a team-oriented environment.
Preferred Qualifications:
? Familiarity with electronic health records (EHR) systems.
? Additional certifications related to geriatric nursing or long-term care.
