Access Services is looking to welcome an Assistant Coordinator to our Lifesharing program. The home base for this role is at our main office in Fort Washington, PA and requires regular travel in the surrounding Delaware Valley region.

You will be part of a team working to support adults with intellectual disabilities/Autism who live in our Lifesharing provider host homes. Our team works with the provider families and the individuals to ensure the home meets the needs and preferences of the person served. As an Assistant Coordinator you support our team of Coordinators, carry a reduced caseload, and transport individuals that live in Lifesharing homes.

A successful Lifesharing Coordinator will:

  • Be organized, detail-oriented, and have a willingness to learn the tech skills needed to use the systems we work in
  • Be confident in their ability to be a team player and collaborate with the other coordinators in the department
  • Be a responsible, out-of-the-box thinker who cares for the well-being of the people we serve and is willing to learn and advocate for them
  • Manage all relationships related to the case and exhibit emotional intelligence while working with the provider's different communication and emotional styles with the goal of maintaining the individual's placement in their provider home
  • Care about people while also strongly prioritizing accuracy and details
  • Have crisis management skills and patience

A week in the life of a Lifesharing Coordinator might look like:

  • Transporting and accompanying our individuals to programs and appointments
  • Being willing to step into the needs of the current day, as every day can look unique
  • Weekly meetings with the Lifesharing department
  • Meetings with the individual's team, their provider family, and county support coordinators
  • Monitoring in the Lifesharing provider's home
  • Managing the individual's medical care and prioritizing the volume of paperwork that is required with regulations and deadlines in mind

Essential Functions

1.   Assist with the completion or coordination of a comprehensive assessment for each individual  being referred. 

2.   Assist with the development, implementation, updating and review of individual support plans  that are strengths based and person centered in collaboration with the team and the  

individuals being served. 

3.   Assist with the coordination of the implementation of all supports outlined in each individual’s  plan.

4.   Assist or facilitate regular team meetings for the planning and review of services, as required. 

5.   Meet with the individuals being served and their families, at a minimum quarterly, to evaluate  service needs. 

6.   Collect and prepare program and/or individual related data as requested. 

7.   Participate in the selection process, orientation, evaluation and supervision of assigned direct support professionals as requested. 

8.   Promote, support, and encourage current trends and/or philosophies within the human  

services field in accordance with Access Services’ philosophies. 

9.   Provide direct care support to individuals when assigned including day, evening and weekend hours. 

10. Maintain service notes, progress notes, and other documentation as requested. 

Requirements

Requirements and Qualifications

Candidates must meet all qualifying and ongoing job criteria including background checks, physical examination results, and clearances.

Education and Experience:

  • Bachelor’s degree from an accredited college or university plus two years of work experience working directly with persons with IDD, or,
  • an Associate’s degree or 60 credit hours plus four years of work experience working directly with persons with IDD, or,
  • high school diploma plus six years of work experience working directly with persons with IDD required. 

Knowledge, Skills, and Abilities:

  • Excellent organizational and oral/written communication skills.
  • Excellent interpersonal skills to maintain effective working relationships with individuals, their families and individuals from local agencies.
  • Ability to effectively interact as part of a team.
  • Ability to work independently.
  • Knowledge of basic family dynamics. Proficiency in, or ability to learn and use, workplace applications, platforms and technologies.
  • Ability to navigate network drives and basic hardware use and troubleshooting.

Driving Requirements:

  • A valid driver's license, and access to a reliable vehicle during work hours.
  • A U.S. driver’s license for at least two years (not including time with a permit).
  • Must be listed as a driver on a valid auto insurance policy.

  • This position requires the ability to travel during the course of a normal workday to successfully carry out the expectations of this position.  Therefore, employee will need to drive their personal vehicle and/or an agency owned vehicle to meet these expectations. Employee is expected to drive in a safe manner at all times, meet eligibility criteria outlined in our Motor Vehicle Policy, be available and willing to drive during work schedule, and successfully pass the agency’s substance abuse screening at any time during employment.

Essential Working Conditions/Physical Demands: Work in program sites and the community routinely providing direct care and support with individual(s). Stand, sit or run by self, or to provide proper care and supervision with an individual. Able to move and stay with an individual to keep them safe, both in home environment and community, both at near and far distances, even when an individual may struggle to meet identified expectations. Able to assist an individual who has fallen, regardless of weight or size. Walk, climb stairs by self, or to provide direct care ensuring individual’s safety.

Compensation and schedule:

  • Starting at $20.75/hour, with adjustments made for education and experience; plus a competitive benefits package
  • Work Schedule: Monday through Friday 9:00AM-5:00PM, flexibility with schedule and on-call rotation required. The location rotates from the Fort Washington office to locations throughout the Delaware Valley region where you will transport individuals who live in our Lifesharing homes.
