Clinical Care Manager/MDS Coordinator

Up to $2,500 sign-on bonus
As the Clinical Care Manager/MDS Coordinator, you'll be responsible for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process in the skilled nursing facility. Adhere to the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) requirements. Encourage each resident to maintain or attain the highest practical physical, mental, and psychosocial well-being, through Community programs and procedures in compliance with applicable laws and regulations, ABHM mission and core values. You'll ensure that medical and clinical care is administered with the residents' wishes, including the implementation of advanced directives. Consistently demonstrate clinical competency and maintains professional knowledge consistent with contemporary standards of clinical practice.
- Salary starts at $72,800/annual and goes up based on experience

What You Need:
- Active, unencumbered IA Registered Nurse license
- Min 2 years skilled nursing experience required
- Certified in ICD-10 coding
- Demonstrate a positive attitude, self-awareness and emotional maturity to manage behaviors in a manner that promotes healthy, respectful and supportive communications
- Demonstrated professional leadership abilities which include active listening, conflict resolution, constructive feedback, coaching, and willingness to develop others to their fullest capabilities
