**URGENT**Adiv Staffing is looking for a compassionate and dependable Clinical Manager to be a part of our amazing team. The individual should be able to work independently, make judgments based on assessments and data available and act accordingly. Must be flexible, innovative and possess good interpersonal skills. Clinical Manager


Responsibilities:


  • To provide Hospice care to the terminally ill hospice patients twenty-four hours/day, seven days/week.
  • To work as a member of the Hospice team in providing Hospice care.
  • Overseeing and supervising all Hospice care to patients and families in all settings.


Qualifications:


  • Graduate from an accredited school of nursing


  • Currently licensed as a registered nurse to practice in the state of GA.


  • 5+ years of experience as a registered nurse.


  • 2+ years of Hospice, Home Health, or Palliative Care Management experience.

  • Great communicator, hard working, positive and energetic.
  • Passionate about hospice and/or end of life care.
  • Ability to work independently, make accurate, and at times, quick judgments
  • Ability to supervise others appropriately.
  • Ability to respond appropriately to crisis outside of a hospital setting.
  • Acceptance of and adaptability to different social, racial, cultural, and religious modes.
  • Ability to reliably commute or ability to relocate prior to starting work.


Documentation:


  • Active RN license in the State of GA
  • A BLS (Basic Life Support) Certification is preferred
  • Valid driver's license with valid auto insurance for the reliable vehicle they will use during work hours


Working Conditions:


  • The individual must be able to tolerate exposure to elements including, but not limited to, odors, blood, body fluids and excrements, adverse environmental conditions, and hazardous materials.
  • This position requires considerable physical effort most of the day including kneeling, squatting, reaching,twisting, climbing, walking, exposure to temperature and humidity changes and maximal assist in lifting and/or transferring of a 100-pound patient. Individual must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position and also provide evidence of annual TB test and other state-required tests or examinations.


Essential Functions:


1. Supervise the planning, coordination, and delivery of the interdisciplinary group interventions.

2. Assign patient/family to appropriate staff member(s).

3. Participate and provide leadership in the team for regular interdisciplinary group meetings; the development, review, and revision of the patient/family plan of care; facilitation of problem-solving sessions; and delivery of team support.

4. Implement and monitor compliance with policies and procedures.

5. Serve as a member of the management team to address problems associated with the delivery of services and staff development needs, to put forth suggestions for appropriate additions/changes in policies and procedures and to give peer input to enhance and improve the management function.

6. Supervise the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations, certification standards, legal and ethical standards.

7. Provide supervision of support and clerical staff in cooperation with other members of the management team.

8. Oversee clinical supervision of nursing personnel, Hospice Aides and Homemakers.

9. Conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members.

10. Provide information for inclusion in written performance evaluations for all staff assigned to team in cooperation with appropriate members of the management team.

11. Provide leadership and counsel to nursing personnel to measurably enhance staff development.

12. Identify need for staff education and assist in the development of in service programs for interdisciplinary group.

13. Assist in education/training programs.

14. Participates in developing standards which ensure safe and therapeutically effective service to patients and families. Has joint responsibility with the Administrator for seeing that standards are met.

15. Participates in developing objectives for Hospice.

16. Is responsible for seeing that objectives are implemented.

17. Consults with the Administrator to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of personnel.

18. Is responsible for recruiting, hiring, evaluating, and terminating service personnel.

19. Participates in planning for the orientation of new employees. Conducts selected orientation classes.

20. Plans and arranges for consultation needs of staff; prepares and maintains current policies and procedures which meet Medicare, Medicaid, accreditation standards, state, and federal laws, etc. and implements; revises concurrently.

21. Displays a willingness to support the policies and procedures and uses appropriate channels for change of such policies; establishes criteria and procedures for selection, promotion, and termination of employment of service personnel.

22. Participates in establishing the functions and qualifications for each service position; coordinates interdisciplinary team services:

• Assures ongoing assessment of patient/family needs and implementation of interdisciplinary group plan of care.

• Assures physician approval of plans for continuity of medical care.

• Provides individual or group support concerning job related stress or issues.

23. Organizes Hospice to delineate and delegate authority, functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.

24. Participates in coordinating Hospice's services with services of other community agencies.

25. Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation, and job performance.

26. Observes confidentiality and safeguards all patient related information.

27. Accepts responsibility for regular attendance and punctuality, fulfills job requirements without regard to time involved.

28. Serves as a resource person to employees.

29. Develops a cooperative relationship and communicates effectively and professionally with the physicians.

30. Investigates and reports any problem relating to patient care or conditions which might harm the patient and/or employee well-being.

31. Immediately reports any accident, incident, lost articles, or unusual occurrence to the   Administrator.

32. Attends pertinent continuing education programs other than routine in services and shares information with staff.

33. Works with Administrator in identifying budgetary requirements and determining appropriate use of allocations.

34. Monitors equipment for appropriate use and take steps to keep misuse to a minimum.

35. Oversees Hospice's ongoing Quality Assessment/Performance Improvement

36. Provides 24 hour/day, seven (7) days/week on-call coverage.

37. Plans and supervises the Hospice.

38. Ensures that patients’ plans of care are developed, implemented, and evaluated.

39. Conducts patient case conferences, in services, staff meetings and maintains documentation; participates in community education projects.

40. Ensures that all necessary supplies and equipment are available.

41. Is normally available at all times during and after operating hours; may designate a qualified temporary replacement if he/she will not be available.

42. Ensures compliance with federal, state, and local rules, accreditation standards and Hospice policies in all patient care aspects of the Hospice.

• Oversees staffing and assignments.

• Addresses and corrects all patient concerns.



Pay: $90,000.00 per annum
