Compensation: $27.96-$44.66 (DOE)
Location: Central Oregon.
Schedule: Monday-Friday 8:00 am-5:00 pm (full-time).
Travel: Local/regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon. Mileage reimbursement is provided.

What You Get to Do:

  • Perform initial and ongoing psychosocial, emotional, and spiritual assessments of patients/families with a holistic perspective that incorporates identifying support systems, coping mechanisms, and barriers to care.
  • Utilize therapeutic techniques to support patients and families in understanding their options and participating in decision-making around their health needs.
  • Provide emotional and spiritual support counseling and complex care management services to palliative care patients and families.
  • Facilitate care coordination and make appropriate referrals based on identified needs.
  • Perform advance care planning, including end-of-life conversations, clarification of medical care goals, discussion of hospice care, and appropriate bills.
  • Assist with patient and family education, including health literacy, cognitive abilities, communication patterns, challenges around their medical care, and learning styles.
  • Engage patients, hospitals, facilities, and the community in education about palliative care and services provided.
  • Work collaboratively with palliative care providers to ensure appropriate follow-up for patient needs.
  • Work collaboratively with community resources to support patients and help ensure continuity of care across service lines and settings, advocating for patient-centered care.
  • Assesses and supports patients and families regarding anticipatory grief and explores family systems/dynamics and coping methods. Evaluate the need for bereavement counseling or appropriate interventions related to complicated grief.
  • Facilitate appropriate and measurable goals to help patients achieve their desired quality of life.
  • Achieve job performance goals, progress toward individual/personal development goals, and demonstrate Partners In Cares values and competencies.
Required Skills & Qualifications:
  • Master of Divinity (MDiv) or Master of Arts (MA) in Theology, plus Clinical Pastoral Education or equivalent pastoral counseling skills.
  • Prefer four units (1,600 hours) as accredited by the Association for Clinical Pastoral Education (ACPE), the United States Conference of Catholic Bishops Commission on Certification and Accreditation, or the Canadian Association for Pastoral Practice and Education (CAPPE/ACPEP); one of these units may be an equivalency.
  • 1-3 years of applicable experience in end-of-life care in a healthcare setting.
  • Board Certification for Chaplaincy or eligibility within three years of hire
  • Specialized certification in palliative care social work or chaplaincy is preferred.
  • Ability to pass a background test, drug screen, and TB test.
  • Valid CPR Certification.
  • For those working in the field, current automobile insurance, a valid Oregon drivers license, and reliable transportation are required.
  • Knowledge of Federal, State, and Local regulations, Medicare Conditions of Participation, and hospice Medicare benefits.
  • Knowledge of HIPAA privacy rules and regulations.
  • Knowledge of electronic medical records systems.
  • Demonstrate excellent organizational skills and ability to prioritize and meet deadlines.
  • Ability to work flexible hours, if needed.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
