About the Role:
We are seeking an experienced Social Worker to oversee the social services program at a long-term care facility in Flagstaff, AZ. This is a unique opportunity to work with the geriatric population and make a significant impact on their quality of life.
Key Responsibilities:
- Develop, implement, and manage social service programs tailored to the needs of the elderly residents.
- Conduct assessments to identify the social and emotional needs of residents and create individualized care plans.
- Collaborate with healthcare professionals, families, and other stakeholders to ensure comprehensive care for residents.
- Provide guidance, support, and training to social services staff.
- Ensure compliance with federal, state, and local regulations related to social services in long-term care settings.
- Facilitate support groups and educational programs for residents and their families.
Qualifications:
- MS in Social Work with at least one year of supervisory experience in a long-term care setting.
- Strong understanding of the unique needs of the geriatric population.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Strong organizational and leadership skills.
