SUMMARY DESCRIPTION OF WORK
The Community Health Worker (CHW) will be based out of Heartland Family Service in Omaha and will play a key role in supporting the screening, assessment, and care navigation for community members enrolled in our Path Assist program. This is an exciting role that will help accelerate local change happening in your neighborhood to drive toward better and more equitable community health.
Compensation: between $17.95 and 21.07 per hour (wage is determined by number of years of experience)
Work Schedule: 37.5 hours per week

MINIMUM QUALIFICATIONS
- High school diploma or equivalent required.
- Two to three years of relevant work experience providing direct care coordination services to individuals and families (Preferred)
- Valid driver’s license, acceptable driving record.
Essential Duties and Responsibilities
- Have a passion for and experience working with individuals and families to make sure they have the knowledge, support, and resources needed to meet their social and health needs.
- *Works directly with member to assess holistic health care and social service needs, Client strengths, abilities, and desires.
- Have experience successfully creating client or patient-centered action plans with community members and connecting them to services and resources from local nonprofits and social service organizations.
- * Demonstrates ability to effectively assist clients in addressing barriers by utilizing community resources and health related information.
- *Coordinates services, resources, information with client, and on behalf of client when needed.
- *Maintain all reports required of the position.
- *Interacts with primary care providers, behavioral health providers and other social service providers to make needed connections for services, provide a conduit for information sharing and organizes case conferences to bring providers together to review the care of mutual members.
- Have experience utilizing electronic platforms to document patient or client care and interactions, adhering to excellent data collection standards.
- Provide care coordination and resource navigation to an assigned caseload of community member clients with unmet social needs.
- Conduct consistent intake, follow-up, and coaching to clients to assist with enrollment in community services/benefits/programs for which they are eligible.
- Administer social determinants of health (SDOH) screening, intake forms, and any needed assessments in the platform.
- Assist clients with prioritizing goals and creating client-centered care plans.
- *Works within the team model, utilizing the expertise of the team members when appropriate for each client.
- *May include teaching life skills or budgeting classes to clients.
- *Provide problem solving and crisis intervention to clients.
- *Provide referral and coordination for issues which may interfere with therapy attendance, including making follow-up phone calls to clients who miss their appointments.
- *The ability to communicate positively with community members regarding coordination of services.
- *Display a comfort level in working with stressful individual/family dynamics.
- *Always displays a courteous and caring attitude to the clientele, volunteers, and visitors of the agency.
- *Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
- *Is dependable and punctual regarding scheduling and attendance.
- Abides by all specific program and Agency procedures, policies, and requirements.
- Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
- *Creates, maintains, and shares as appropriate a dynamic self-care plan.
- *Strives to make connections between the agency and the larger community whenever possible to contribute to the agency’s ongoing fundraising and friend-raising efforts.
- *Essential functions of this job is to be performed on company physical work site.
- Performs other program related duties as assigned.
