As we continue to expand our reach, we are looking for a skilled and experienced Hospice Community Liaison to join our team and advocate for our hospice service.

As the Hospice Community Liaison at Health Link Home Health and Hospice, your primary responsibility will be to leverage Health Link's extensive relationships in the community to build trust in our hospice program.

Hospice is about trust - in order for nurses, social workers, and doctors to trust us with their patients we must keep our promises and live out our values. Your responsibility is to be the front line of our hospice team and give more patients the opportunity to be cared for by our amazing clinicians.

You will work with the Director of Marketing and Hospice leadership team to execute a sales and marketing plan and achieve our goals. To be successful in this role, you will need a keen understanding of the hospice industry, as well as a solid understanding of marketing and sales. You will need to digest information rapidly and learn as much as possible as fast as possible.

Health Link has been a trusted community partner in the Bay Area for 18 years but we still operate like a startup. If you focus, buy in, keep your promises, and work hard you will be extremely successful.

Responsibilities

  • Implementing a comprehensive marketing / sales plan for Health Link Hospice
  • Develop and maintain new and current relationships with community partners including hospital CM / SW teams, doctor office and clinic teams, senior living communities, and skilled nursing facilities.
  • Ride alongs with current team members, respecting existing relationships, and developing your own.
  • Calls to schedule meetings, visits to present our services, and a TON of follow up.
  • Collaborate with the hospice leadership team to coordinate referrals and be the liaison between our community partners and internal staff. Lots of messaging and coordination calls.
  • Represent Health Link Hospice at relevant community events and meetings to build awareness, develop relationships and build our network.
  • Manage your own budget, including tracking expenses and ensuring effective use of resources.
  • Identify, target, and develop new relationships into reliable referral partners.
  • Perform Hospice education sessions with patients and families to learn their goals & preferences while simultaneously evaluating if they would be a good fit for our services.

Requirements

  • A passion for hospice
  • 1 year of experience in hospice marketing
  • Strong understanding of the healthcare industry, market segments, and best practices.
  • The ability to stay organized with a CRM, calendar, and manage multiple opportunities and pipelines.
  • Excellent communication and interpersonal skills, with the ability to build and maintain relationships.
  • Track record of hitting your targets and achieving your goals.
  • Ability to work independently and as part of a team, with strong project management skills.
  • Valid driver's license and clean driving record.
  • Bachelor's degree
