We are a growing, fast-paced physician-led office that is a leader in our field of treating kidney disease looking for a full-time Advanced Practice Provider. The right candidate will thrive in a multi-tasking environment serving our patients and serve as a partner with our team of excellent physicians and APPs. A few of the key duties of this role will include comprehensive management of patient conditions including assessment, diagnosis, treatment, and consultation, on a longitudinal basis of chronic kidney disease patients. We are partnered with Evergreen Nephrology and their interdisciplinary teams to support our managed care approach with our patients. The majority of time spent in this role will reside in our CKD offices and assigned dialysis unit (including some travel to satellite offices).


We offer a generous paid time off plan, eight paid holidays, no on-call afterhours or weekends, a full compliment of medical, dental, vision, life and disability insurance, CME, mileage and phone reimbursement along with an unparalleled retirement plan. The salary range for this position is commensurate with candidate experience.

Completion of an accredited program for Nurse Practitioner (NP). Nephrology experience preferred but not required.
