Home-Based Family Support Specialist

As a Home-Based Family Support Specialist at Greater Seacoast Community Health, you will deliver effective home-based, family-centered services to expectant women and families with children up to the age of 18, in compliance with Greater Seacoast Community Health policies and state contracts. Your role involves providing essential parenting, child development, health, and safety information to enhance family well-being and reduce risks for child abuse and neglect. You will develop Individualized Family Support Plans, document activities for billing and case records, and coordinate services with Greater Seacoast Community Health, social service agencies, and medical practices. Enjoy varied and engaging workdays.

Job Status: Full-Time (Non-Exempt)

Hours: Monday through Friday, 8:00 AM - 5:00 PM

Salary: $26.00 per hour

Key Responsibilities:

  • Deliver parenting, child development, health, and safety information to families in compliance with Families First policies and state contracts.
  • Develop and implement Individualized Family Support Plans (IFSPs) for families.
  • Document activities for billing and case records accurately and timely.
  • Coordinate services with Greater Seacoast Community Health staff, social service agencies, and medical practices.
  • Support families in improving access to health resources and social services.
  • Provide case management and utilize strength-based approaches when engaging with families.
  • Maintain confidentiality, professional boundaries, and ethical practices in all interactions.
  • Participate in multidisciplinary team meetings to ensure comprehensive care for families.

Qualifications:

  • Associate’s degree in social work, counseling, human services, early childhood education, or a related field required.
  • Minimum two years’ experience working with families and children in a social service, home health, or early childhood program setting.
  • Experience in case management and family support principles.(Preferred)
  • Strong computer skills.
