The Case manager / Resident Guide is responsible for assisting residents with case management needs three days a week & resident guide responsibilities two days a week.
The essential job functions of this position include managing moving pieces of individual resident appointments & transportation while progressively empowering them to meet their needs independently, working closely with multidisciplinary team, orienting resident to the program including all necessary paperwork, assisting residents in setting and meeting weekly goals based on program phases and areas of growth, overseeing the operation of the home taking responsibility for integrating and administering all operations, caring for resident needs between program hours with understanding and care, maintaining a calm and comfortable setting within the house.
