PURPOSE/SUMMARY:

Through direct service provides for the spiritual needs of hospice patients, families, staff and volunteers. It may involve mobilizing area clergy and lay religious leaders.


SUMMARY OF ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES


SPIRITUAL ASSESSMENT AND CONSULTATION

  1. Ensures that patient/family spiritual needs are met and assists the Interdisciplinary Team (IDT) members in spiritual needs and issues.
  2. Assesses and/or facilitates team assessment of patient/family spiritual needs as related to the terminal illness process.
  3. Completes Initial Spiritual Assessment and spiritual Plan of Care (POC) for each patient/family requesting counseling within 5 days of admission.
  4. Maintains a collegial relationship with community religious and spiritual representatives both lay and clergy.
  5. Integrates spiritual issues into the plan of care, evaluates effectiveness of interventions and initiates follow-up action as needed.
  6. Provides consultation and education to interdisciplinary team members and others regarding spiritual issues and needs of patients, families and caregivers during the terminal disease process, in cooperation with other team members.
  7. Must meet visit expectations of 8-10 (eight to ten) visits per week.
  8. Ensures that the spiritual concerns of patients/family members, staff and volunteers are addressed through direct services or community clergy referral.
  9. Contributes to the hospice Plan of Care for each patient.
  10. Provides support and assistance for the spiritual care of the patient/family/caregiver.
  11. Officiates or assists in coordinating religious rituals and funeral services as requested by the patient/family.
  12. Responsible for the organizing of periodic patient memorial services for staff and patient’s families.
  13. Assists Bereavement Services in the annual memorial service for staff and patient families.
  14. Provides patient care education to families, facility staff and community as needed.

INTERDISCIPLINARY TEAM/COMMUNICATION

  1. Ensures continuity of care to patients/families by effectively collaborating and communicating with IDT members and the patient/family/caregivers.
  2. Attends IDT weekly meetings and actively participates in discussions related to patient care.
  3. Collaborates with IDT members regarding the patient/family plan of care to ensure continuity of care and the inclusion of spiritual issues.
  4. Recognizes the knowledge and skills of other disciplines represented on the team.
  5. Provides in-service training to hospice staff as needed and requested.
  6. Ensures the patient’s/family right to confidentiality in all communications.


DOCUMENTATION AND RECORDS

  1. Ensures compliance with YoloCares’ policies and procedures, Federal and state regulations, and current Hospice standards.
  2. Documents in the patient records for each contact with patient/family/caregivers.
  3. Contacts each patient/family admitted to YoloCares within 5 days of admission.
  4. Completes Spiritual Assessment for each patient within 5 working days of the admission.
  5. Participates in the agency Quality Assurance/Performance Improvement program.
  6. Maintain and update the area Clergy Listing.


Requirements:

JOB QUALIFICATIONS

1. A minimum of four quarters of Clinical Pastoral Education training, or comparable experience is required.

2. Excellent interpersonal, oral and written skills.

3. Ability to communicate with people of all religious/spiritual backgrounds without enforcing personal views.

4. Knowledge of the social and spiritual dimensions of terminal illness and of the Hospice concept.

5. Skilled in clinical assessment and ability to participate in patient care planning.

6. Ability to do work compatible PC software computers is preferred.

7. Knowledge of abuse/neglect report requirements and other significant regulations affecting clinical practice are required.

8. Ability to successfully meet agency background screening requirements.

PHYSICAL & OTHER JOB SPECIFICATIONS

1. Able to hear, speak, read, write, and type well enough to perform required job duties.

2. Able to lift up to 15 pounds without physical problems.

3. Ability and willingness to maintain and drive a personal vehicle. A valid California driver’s license and state-required automobile insurance, are required.

4. Must be willing and able to flex hours to meet occasional appropriate business needs during evenings or weekends.
