Job Title: Registered Dietitian

Supervisor: Center Manager

FLSA Status: Exempt

Job Summary: Responsible for providing nutrition service and education to PACE of the Southern Piedmont participants in a home and community-based model of care. Responsible for evaluating the participant’s nutritional status, development and implementation of nutritional care plans, assessment of nutrient requirements, recommendation of nutritional prescriptions, evaluate participant tolerance, effectiveness of nutritional care, and communication of nutritional data and information to the Primary Care Providers and other health professionals. This position reports to the Center Manager/Healthcare Coordinator. Reports Directly to the Center Manager.Specifications

Education: Bachelor’s Degree from ADA accredited institution with a major in nutrition. Master’s Degree plus approved work experience preferred.

Experience: 1 year of experience working with the frail, elderly population.

Number and Type of Employees Supervised: none

Licensure, Registry or Certification Required: ADA registered. CDE status preferred but not required.

Special Training: Requires ability to teach; analytical thinking; and ability to provide care to geriatric participants; knowledge of clinical dietetics. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE of the Southern Piedmont and approved by CMS before working independently.

Immunizations: Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact

Neonate/Infant Early Childhood Adolescent X Adult X Geriatric All Age Groups

Key Responsibilities: (*denotes an age-related skill or task)

  • Performs nutritional assessments and reassessments as indicated by nutrition standards.
  • Provides nutrition education to participants/caregivers as ordered by physician and according to ADA standards. Provides nutrition education to staff and the community.
  • Works collaboratively with other team members implementing participant education.
  • Assists with coordination of 24-hour care delivery.
  • Documents participant changes appropriately in the medical record and communicates participant changes to IDT as needed.
  • Updates participants’ care plans appropriately throughout the reassessment period.
  • Attends and participates in staff meetings.
  • Participates, collaborates, and contributes as a member of the Interdisciplinary team (IDT). Emphasizes teamwork and collaboration in all clinic and Interdisciplinary interactions.
  • Supports his/her Interdisciplinary Team and promotes unity among the team while interacting with the team, other co-workers, and/or participants.
  • Participates in participant care planning including the implementation of SMART goals/interventions for the participants’ care plans and enters all care plan information in a timely manner as per organizational protocols.
  • Accepts teammates’ differences in professional assessment findings, as they relate to participant care, and supports teams’ decisions while promoting a united team for the participants’ continuity of care and for staff development.
  • Supports Senior TLC’s mission to encourage and support the quality of life of seniors wishing to continue living in the community; its vision to be the preferred provider of individualized care for seniors in the community; and its values of respect, integrity, accountability, compatible goals, and compassionate care.
  • Reviews and approves all menus.
  • Procures food, including nutritional supplements from sources approved or considered satisfactory by Federal, State, or local authorities with jurisdiction over the service area of the organization.
  • Supervises the activities of the dietary department daily.
  • Abides by the food safety and sanitation practices required by the Commission for public health rules applying to Adult Day Health care facilities.
  • Other duties as assigned by supervisor.

Participant Interface: Must be comfortable and eager to engage our participants in a warm and welcoming manner while maintaining a professional distance and attitude. Experience with issues of aging and health challenges would be helpful. Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
