Position Summary

The Maternal and Child Doula/Community Health Worker will collaborate with the Certified Nurse Midwife, Pediatric provider, and clinical staff to identify needs, organize activities, and coordinate care for patients.

Main Responsibilities

  • Assess the health status of patients
  • Develop and maintain care plans for patients with the medical providers
  • Provide care management for the patients

Qualifications

  • 3-5 years' experience in health care setting, preferably primary care (doula, OB/pediatrics, medical interpreter and/or community health worker)
  • Associate degree or Bachelor's degree preferred
  • Bilingual: Somali-English preferred
