Assignment Duration: 3-4 months
Start: as soon as credentialed
BID DUE DATE: 8/21/24 4p cst
Total # FTE s needed: 1-2
Rate Caps for Submission:- Malpractice Must be included in rates.
- Orientation half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
- Hourly xx351-375 - Per hour, not OT
- Holiday Must be 1.5x the hourly rate. Per holiday hour, no OT
- Nightly Call $701-900 - Per night call to hold the pager, to include X hours gratis
- Holiday Nightly Call - "Per night call to hold the pager, to include X hours gratis
- On Call (Daily) $2901-3200 - Per 24 hours to include 4 hours gratis"
- 24 Hour Holiday Call Rate - Per 24 hours to include 4 hours gratis"
- Call Back $351-375 - Must match Hourly rate Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.

Minimum length of commitment required: 3 months
Minimum number of shifts preferred/month: 5-6 days of call plus clinic

Duties: general urology care in clinic and on call, need a bread and butter urologist for routine care
Required Procedures: bread and butter urology, robot available
Patients per day/volume: 20-25 pts per day
Number of surgeries: 20-30/month

Admissions: Hospitalist team present

- Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where the provider will be traveling from in the travel section of the provider submission.

Credentialing with other facilities? locums automatically credentialed at all facilities not including St. Michaels.

Vendors are now required to compile all presentation documents for a locum into a single PDF file.
The PDF must be uploaded into TRIO in the "Candidate Documents" section under the locum's submission page.
The PDF title should include the locum's name, site location, and specialty (ex. Dr. John Smith Good Samaritan Physician Nephrology)
An up-to-date CV (solely) is still required to submit in Name Clear Documents initially when sending a new name clear. Vendors will have until bid due date to submit a PDF of ALL presentation documents. If PDF is not submitted, TRIO will not clear provider.

Malpractice documentation (NPDB and or write up). Must include ALL cases, dates, and payouts.
