9am-5pm (2) shifts/week, per diem weekend rotation, holiday rotation.

The LPN Coordinator (for the Admissions Department) handles the day to day coordination of the team clinical functions, assists in implementing the patient plan of care with staff, volunteers, and outside agencies. Communicates as a liaison between staff, patients, and outside agencies. Assists with maintaining records. May involve direct patient care.

Essential Duties and Responsibilities for the LPN Coordinator:

  • Assists in coordination and implementation of the patient plan of care.
  • Handles phone calls from the patient, primary care giver, or designee regarding any questions or concerns.
  • Collaborates with the interdisciplinary teams, RNs, or Team Manager.
  • Coordinates and schedules patient visits.
  • Provide timely documentation/data entry per Hospice time frames and deadlines.
  • Communicates current health status and needs of patients to appropriate team professionals.
  • Assists the team and Team Manager with other administrative functions, as necessary.

Qualifications for the LPN Coordinator:

  • Education/Certification
  • Graduate of an accredited School of Practical Nursing with current LPN licensure in NYS.

  • Experience
  • 1 year of nursing practice as a Licensed Practical Nurse preferred.
