The Housing Case Manager for the HEARTH (Helping to Empower and resettle through housing) Program is responsible for guiding families exiting shelters into their new permanent housing and supporting them through stabilization as they move toward self-sufficiency. The Case Manager provides assigned families with stabilization services, including connecting them to necessary resources such as language training and career counseling, and orienting them to their responsibilities as tenants. The Housing Case Manager will work collaboratively as a member of the Housing Team with a focus on housing, stabilization, future planning and goal setting.

RESPONSIBILITIES

  • Review program policies and procedures to ensure compliance.
  • Adhere to all privacy policies and procedures related to the security of Personally Identifiable Information and Protected Health Information for all clients.
  • Ensure all services are provided in the native language of the client; use designated interpreter or translation services as needed to have effective communication.
  • Together with the Housing team, develop customized permanent housing plan. Educate families on how to be active participants in the housing search.
  • Assist with apartment/home setup and ensure required furniture, household and personal items are in home prior to move-in date.
  • Transport or arrange for transport of family to permanent housing. Conduct safety orientation to new home and neighborhood within twenty-four hours of placement.
  • Assist the family to open accounts with local gas, electric and waste companies as needed.
  • Complete all necessary procedures upon placement of a family in housing by the HEARTH program, including tenant expectations under the lease and housing safety protocols.
  • Assist with Housing team to ensure positive relations with landlords and property managers.
  • Conduct regular case management meetings with adult members of the family to ensure progress towards self-sufficiency.
  • Comply with MORI guidelines for family stabilization after placement into permanent housing. Conduct home visits to ensure compliance with guidelines, agency expectations, and lease terms.
  • Complete all client and program required reports and submit within specified timeframes.
  • Maintain orderly case files and comprehensive documentation on client services and referrals. Ensure that all information reported is accurate, documented, and reliable.
  • Work environment includes an office environment as well as interacting with clients in the shelter system with frequent driving to meet with clients.
  • Maintain professional boundaries and confidentiality at all times.
  • Attendance at all internal and external meetings and training as required.
  • Local travel as needed
  • Other responsibilities as assigned.

QUALIFICATIONS

  • Bachelor’s degree with a concentration in human service or related area.
  • Experience working with refugees, homeless populations, and other vulnerable populations preferred.
  • Bilingual English - Haitian Creole or English-Spanish a plus.
  • Good listening skills with proven de-escalation skills.
  • Ability to work independently and as a team player to accomplish program goals and assist clients.
  • Cultural responsiveness, commitment to racial justice, and ability to relate well and effectively in a multicultural and multilingual workplace with diverse team and client population.
  • Strong outreach, communication, and relationship building skills.
  • Good critical thinking skills.
  • Strong organizational ability with attention to detail and multi-tasking skills.
  • Ability to maintain accurate, comprehensive records.
  • Competency with Microsoft Office Suite including Word, Excel, PowerPoint and Outlook and virtual meeting platforms such as Zoom, Teams and other technical platforms.
  • Willingness and flexibility in learning new functions.
  • A valid MA driver’s license with a good driving record and access to a reliable car are required.
  • Ability to work evening and weekends as needed.
