Workforce Case Manager

As a case manager, you will engage consumers in the interactive process to help them identify needs, barriers, and goals in order for them to become prepared and qualified for employment.
ESSENTIAL DUTIES:
- Develop positive relationships with consumers to help them identify and overcome potential limitations/barriers to attain training or employment.
- Access consumers work readiness, strengths, skills, aptitude and training/occupational goals.
- Develop an effective Customized Employment Plan or Recovery to Work Plan to help consumers overcome barriers and obtain skills, training, professionalism, a valid driver's license, stable transportation, and clean drug screen.
- Utilize all established support services to help consumers develop needed technical and life skills which will lead to successful attainment of good paying careers and employment.
- Maintain all required information and client data within the case management system and provide reports as requested.
- Associate’s or higher in social work or related field or HSD/equivalent and relevant job experience as a case manager working with persons who have barriers to employment.
- Knowledge of persons with criminal histories and/or addictions
- Knowledge of community resources, workforce needs and opportunities.
- Excellent communication skills
- Valid Ohio driver’s license
Will be required to have post offer drug screen and civilian background check.
