Job Description

POSITION TITLE: Qualified Intellectual Disabilities Professional (QIDP)

DIVISION: Community Living

SUPERVISOR: Regional Assistant Director, Community Living

CLASSIFICATION: Non-Exempt

STATUS: Full-Time

PURPOSE:

Integrate, coordinate, and monitor the active treatment program and medical services for each client receiving services. Represents the client to ensure that the individual receives needed services and interventions from providers who deliver quality services. Maintain person-centered plans.

KEY RESPONSIBILITIES:

1. Write training plans, and case conference agreements for clients in waiver programs. Following Life Course framework model.

2. Develop, write, and implement training plans for clients regarding medical issues, including but not limited to BQIS regulations regarding medications and side effects, seizure plans and precautions, dehydration and constipation, nutritional/dietary needs, and safety/behavioral needs.

3. Provide individual specific training to Managers and provide updated trainings as client needs/requirements change.

4. Attend and participate in quarterly and annual case conferences and other meetings related to clients.

5. Complete and maintain all required paperwork for clients.

6. Review monthly data documentation and write monthly progress reports for clients. Upload to BDDS portal as required monthly.

7. Ensure documentation of specific program areas and services are completed and accurate, including but not limited to daily notes, MARS, training plans, risk plans, and any other necessary documentation.

8. Coordinate the delivery of approved services between selected service providers. Serve as a liaison between clients, families, staff, external professionals, and the community.

9. Coordinate and evaluate the instructional activities that are provided to clients.

10. Complete incident reports, including BDDS, and needed follow-ups within 24 hours of incident notification.

11. Participate in monthly Case Record Review committee meetings and record review follow-up.

12. Provide direct care as needed and participate in the on-call rotation as requested. May perform some staff supervisory functions when on call.

13. Display patience in all situations involving clients. Speak in an even, positive, and personal tone of voice when conversing with clients.

14. Appropriately respond to client’s behavior via verbal and/or physical intervention based on the needs of the clients.

15. Provide services in compliance with all funding sources and governing bodies (CARF, BQIS, State Board of Health, State and Federal Law).

16. Follow policies/procedures set forth by the program department and the agency.

17. Maintain the confidentiality of clients.

OTHER FUNCTIONS:

1. Promote agency philosophies, generate team spirit, create, and maintain an environment that fosters employee satisfaction, maximizes productivity and profitability.

2. Plan and provide instruction in communication skills, daily living, socialization skills, fine and gross motor skills, perceptual skills, prevocational skills, and any other areas based on programming needs, as needed.

3. Assist with planned activities as needed. Ensuring inclusion in community events and activities.

4. Must be able to care for the physical needs of persons served including feeding, hygiene, and toileting. Must be capable of assisting in physically transferring/moving persons served when needed. Physical activities may include but not be limited to: lifting up 25-50 pounds (depending on person served); bending; reaching; stretching; grasping; visual acuity; walking; squatting, guiding; and transporting.

5. Assist with the use of medical adaptive equipment including, but not limited to: Wheelchair, Hoyer Lift, Shower Chair, Blood Sugar Testing Equipment.

6. Assist with the review of billing and time sheets for accuracy and compliance with regulations.

7. Coordinate and transport clients to medical appointments and/or procedures as needed.

8. Assist Leadership as requested.

9. Maintain knowledge of Life Course framework and the tools available.

QUALIFICATIONS:

1. Bachelor's degree in special education, education, behavioral science, or related field from an accredited college or university preferred, or 5 years of experience previous experience working with persons with disabilities preferred.

2. Must possess excellent communication skills both written and oral, and strong teamwork skills.

3. Proficiency in Microsoft Word and Excel.

4. Must possess or be able to obtain CPR and first aid certification annually and perform as necessary.

5. Must possess or be able to obtain NCI certification annually and perform as necessary.

6. Possess and maintain a clear criminal record per Medicaid Waiver regulation requirements.

7. Maintain a valid Indiana driver's license, insurance on personal vehicle (minimum of PLPD with Medical liability coverage), and maintain insurability as defined by the agency liability insurance carrier.

8. Must possess and maintain reliable transportation that meets state requirements for transporting persons served.

9. Must have the ability to successfully work in both independent and group settings. Must have the ability to effectively communicate during a crisis situation.

10. Dress appropriately as defined by department dress code.
