Pharmacy Coordinator Of Patient Outreach

SUMMARY: Communicates with patients over the phone and in pharmacy to address both clinical and non-clinical issues. Coordinates care between the patient, pharmacy, medical provider and outside specialists. Directs issues to other parties involved in the patient’s care. Assists with housekeeping and maintenance of pharmacy. Assists with accurate and timely prescription processing. Responsible for additional clinical and administrative tasks assigned by the pharmacy manager.
SUPERVISOR: Pharmacy Manager
DUTIES AND RESPONSIBILITIES:
Clinical Duties
- Utilize the electronic medical record to create patient appointments and document encounters
- Assess medications for indication, efficacy, safety and convenience and identify medication discrepancies
- Provide wellness education to patients
- Scan patient documents into the electronic medical record
- Collect data to assess patient outcomes
Patient Outreach
- Provide continuous patient care
- Listen to and solve patient problems
- Coordinate care between pharmacy, medical provider, and specialists
- Inform appropriate personnel (pharmacist, medical provider, community health worker) of any patient issues
- Direct patients to area agencies/programs that assist with social determinants of health and health disparities
- Onboard new patients
Assist in the Pharmacy Processes
- Input patient and prescription data into the computer system
- Process 3rd party billing and insurance rejections
- Accurately and timely fill prescriptions
- Refer drug questions to the Pharmacist
- Follow current and new Federal and State Laws
- Ensure prompt service by assisting patients
- Answer telephones promptly
- Complete pharmacy patient transactions on the cash register
- Assist with housekeeping and maintenance of pharmacy
Additional clinical and administrative tasks as assigned by the pharmacy manager
MINIMUM QUALIFICATIONS:
Required experience and education:
- Must be 18 years old or older
- Must pass Background Check- WebCheck (BCI & FBI)
- Must have basic computer skills
- Bi-lingual skills – preferred
- Must have 1-year experience as a senior pharmacy technician
LICENSES OR CERTIFICATIONS REQUIRED:
- Registered as a Certified Pharmacy Technician with the Ohio Board of Pharmacy
- Must have passed either the Exam for the Certification of Pharmacy Technicians (ExCPT) or the Pharmacy Technician Certification Examination (PTCE).
- Certification in Good Standing with either the Institute for the Certification of Pharmacy Technicians (ICPT) or the Pharmacy Technician Certification Board (PTCB)
COMPLIANCE:
This position requires compliance with API's written standards, including its Compliance Program and Standards of Conduct and policies and procedures. Such compliance will be an element considered as part of the regular performance evaluation.
Failure to comply with API's Written Standards, which may include the failure to report any conduct or event that potentially violates legal or compliance requirements or API's Written Standards or, for managers and supervisors, fails to detect non-compliant conduct where reasonable efforts would have resulted in detection, will be met by the enforcement of disciplinary action, up to and including possible termination, in accordance with API's Compliance Program Policy and Procedure: Addressing Instances of Non-Compliance through Appropriate Disciplinary Actions.

While performing the duties of this job, the employee is frequently required to use close vision (clear vision at 20 inches or less). The noise level in the work environment is low to moderate. Stress level can be high at times.
