Resident Care Coordinator

Summary Duties and Responsibilities:
The Resident Care Coordinator (RCC) will assist the Director of Wellness with operational duties and assist with training of employees to ensure proper delivery of resident care. The RCC is also responsible for reporting timely changes in resident has a change in condition and acting as a liaison with community providers. The RCC will perform audit and compliance checks to ensure regulatory compliance obligations are met. The RCC will ensure proper staffing levels through the scheduling of Caregivers and Medication Technicians to include working vacant uncovered shifts to ensure proper staffing to deliver resident care.
Essential Functions and Work Duties:
The following essential functions of this position are performed personally, in cooperation with the supervisor, and/or in coordination with other staff. Additional work functions and duties may be assigned.
- Observe and assist with delivery of care services to maintain awareness of resident needs such as shift task assignments.
- Assist new residents with arranging personal items and help familiarize residents and their family members with the community and services. .
- Assist the Director of Wellness in coordinating incidental medical, dental, vision, hearing and podiatry care for residents and arrange resident escorts as needed.
- Assist Caregivers and Medication Technicians in the performance of their job duties and assume their job duties when needed.
- Ensure proper department scheduling with adequate staffing in accordance with company standards, policies, procedures, budgets, and state regulations.
- Arrange for coverage of job duties within the department as the result of employee absences either through delegation or personal completion of duties and shifts.
- Supervise Med Room processes to ensure timely and accurate medication assistance and communication of resident condition. Conduct weekly Med Room and Med Cart Audit, reporting to Director of Wellness for quality assurance.
- Maintain all resident health records in accordance with regulation.
- Monitor for proper documentation regarding change in condition and incident reporting for both residents and staff. Ensure all paperwork is completed and that proper documentation is maintained in the medical record.
- Assist with training caregiving and medication technician employees regarding medication administration.
- Coordinate services related to resident care needs as requested by the Director of Wellness or Executive Director.
- Alert a Director of Wellness and Administrator when a resident change of condition is noted, or when there is an issue with obtaining a medication for a resident.
- Audit the medication administration records (MARS) and systems to ensure accuracy and completeness, within regulatory guidelines.
- Act as key contact person between communities and long-term care pharmacy.
- Responsible to ensure all medication orders, including refills, are processed, received, and accurate with physician orders.
- Perform frequent medication cart audits to ensure sustained compliance.
- Order and maintain adequate supplies needed to provide resident care services. Report purchases and budget variances to Director of Wellness and Administrator
- Demonstrate proficiency with standard office equipment and MS Outlook applications.
- Ability to adapt and respond to change, make decisions in stressful situations, and prioritize tasks and projects.
- Able to analyze, solve and respond to problems or concerns.
- Must be able to work independently and as part of the community leadership team,
motivating others to meet shared goals.
- Cooperate with, participate in, and support the adherence to all internal policies, procedures, and practices in support of risk management and overall safety and soundness.
- Perform tasks which are supportive of the essential functions of the job, but which may be altered or re-designed upon individual circumstances by your supervisor or licensed Administrator.
- Coordinate specific work tasks with other personnel within the department as well as with other departments to ensure the smooth and accurate flow of information.
- Perform other duties as from time-to-time may be determined necessary for the well-being of the overall operation.
Education and/or Experience:
- Must be eighteen (18) years of age.
- Prefer two (2) years’ experience working in an Assisted Living Facility for seniors providing direct care and have a valid Med Tech Certification.
- Obtain and maintain valid first aid and CPR certification.
- Obtain and maintain a valid Food Handler’s certification, as needed.

Physical Requirements:
- Body agility is important: maintaining body equilibrium to prevent falling when walking, standing, or crouching.
- Lifting: raising objects from a lower to a higher position or moving objects horizontally from position-to-position.
- Stooping - frequently bending body downward and forward by bending spine at the waist, requiring full use of the lower extremities and back muscles.
- Kneeling - frequently bending legs at knees to come to rest on knee or knees.
- Reaching - frequently extending hand(s) or arm(s) in any direction.
- Handling - frequently seizing, holding, grasping, turning, or otherwise working with hand or hands.
- Regularly required to use hand to finger, handle, or feel, reach with hands and arms, and talk or hear using the telephone, computer keyboard, and mouse while seated at a desk.
- Specific vision abilities required by this job include close vision, depth perception and ability to adjust focus.
- Medium work: Exerting up to 50 pounds of force occasionally, and/or up-to 20 pounds of force frequently, and/or up-to 10 pounds of force constantly to move objects.
- The worker is not substantially exposed to adverse environmental conditions.
- Walking: Moving about on foot to accomplish tasks, particularly to deliver mail and packages.
- The worker may potentially be exposed to infectious diseases.
