JOB SUMMARY

Outreach Care Specialist (OCS) will work with value based and managed care health plans, and other payers offering incentive plan initiatives and contracts that use data and health informatics to improve quality of care, achieve better health outcomes, and reduce cost of care. The OCS is responsible for scheduling annual wellness visits and follow up care appointments to adhere to the goals and metrics of the initiative and/or contract. The position will also support the call center during low volume intervals and assist with calls and related duties as needed.

ESSENTIAL FUNCTIONS

• Contact payer assigned patients to schedule annual wellness or follow up care visits.

• Ensure the accuracy of patient information in the electronic health records, updating as necessary.

• Provide any necessary information regarding the appointment or about the organization.

• Communicate any changes in schedules to relevant parties promptly.

• Respond to patient inquiries related to scheduling.

• Answer calls and respond to online appointment requests.

• Accurately schedule patient appointments for all physicians and locations using knowledge of the physician’s specialty, schedule and accepted insurances.

• Enter or verify and update as needed all personal and billing information when scheduling appointments in the electronic health records.

• Obtain identification data, insurance and income information, etc.

• Accurately schedule all appointments with Transportation Requests in coordination with Transportation Department.

• Identify, research, and resolve patient issues. • Educate the patients on appointment scheduling, systems of care, and services offered.

• Recognize, document, and alert the supervisor of trends in patient calls.

• Adhere to all CAHN standards, guidelines, policies and procedures

• Perform other duties as assigned by supervisor


KNOWLEDGE, SKILL, ABILITIES, AND OTHER CHARACTERISTICS

• Excellent interpersonal, analytical, communication skills and customer service oriented.

• Must be computer literate with ability to demonstrate typing speed of at least 40 wpm

• Demonstrates knowledge of various insurance plans and policies

• Team player, goal oriented and highly adaptable

• Ability to work in a fast paced, multicultural environment

• Embrace change and challenges with the ability to provide solutions


EDUCATION AND EXPERIENCE

• High school diploma, GED, or equivalent (minimum)

• One to three years of progressive customer service experience.

• Experience in a call center and/or medical office highly desirable.

• Familiarity with eClinicalWorks a plus.

• Intermediate computer and office skills required.
