SUMMARY: The Director of Community Health and Wellness is responsible for fostering and promoting the vision and strategic direction of the department as it relates to the overall organizational mission, clinic initiatives and the community. The director will be responsible for the implementation of wholistic health promotion initiatives to address the health disparities that affect Esperanza Health Center’s surrounding communities, patients, and staff. This includes: organizational/departmental strategic planning; program development and marketing; participating in management meetings; supervision of department staff; grant writing; tracking income and expenses in regards to specific grants and general department budget; assisting with clinic public relations and attending community meetings; and coordinating partnerships with outside agencies for departmental programs and facility use. The Director will also lead interdepartmental collaborations and initiatives within the organization.

ESSENTIAL FUNCTIONS

  1. In coordination with CHW and clinical staff design, coordinate, and market health promotion initiatives that address health disparities common to Esperanza Health Center’s surrounding communities. Examples include family programming, community health promotion programming, etc.
    1. Assure that program design fits within Esperanza’s mission, strategic planning, community health disparities and community assets, and personnel and financial resources.
    2. Coordinate program implementation so as to maximize program objectives. Problem-solve with and provide feedback to health center administration and other appropriate staff as to program successes and challenges.
    3. Together with Development staff and others participate in grant writing, promote clinic and departmental programs for target audience.
  2. Staff oversight
    1. The Director will provide oversight to all department staff including supervision of the Associate Director and Associate Medical Director.
      1. Ensure the Associate Director accurately completes all departmental timesheets and submits them on time.
      2. Ensure the Associate Director processes and coordinates vacation requests.
      3. Uphold and enforce health center personnel policies.
      4. Evaluate staff performance at the end of the probationary period and annually. Provide accountability, encouragement, mentorship, and leadership to department staff with humility and sensitivity to communal spiritual growth.
      5. With the support of Human Resources or other appropriate health center leadership, implement disciplinary measures with attention to fairness, careful documentation, and a trajectory towards enabling all staff to succeed.
      6. Actively participate in the interviewing process for new departmental hires. Objectively evaluate competencies, congruence with other team members, and alignment with the health center mission.
  3. Budget planning, monitoring and oversight
    1. Submit an annual department budget that is in line with health center priorities, projected income and expenses essential to accomplishing programmatic goals, and avoids waste.
    2. In coordination with the Associate Director and Finance Department, regularly track income and expenses, process check requests, and alert health center administration of any not easily controllable significant current or anticipated trends towards over or under spending as compared to approved budget.
    3. In coordination with the Finance Department and Development team, track grant income and expenses in order to provide a clear accounting of funding spent within given grant parameters at the conclusion of grant term or for interim grant report.
  4. Community Relations
    1. In coordination with the CHW team, attend community stakeholder meetings aligned with health center priorities, as needed. Favorably represent Esperanza Health Center, strategically network, and promote a spirit of comradery amongst individuals, community groups, and agencies serving North Philadelphia.
    2. Be attuned to community perceptions of Esperanza Health Center programs and services. Foster a spirit of humility, listening, and unity in response to any criticisms.
    3. Oversee resident engagement efforts such as the CORE Action Team, and ensure that community voice is brought into departmental decision-making and organizational strategic planning
  5. Partnership coordination and facility use
    1. Identify mutually beneficial partnerships for accomplishing health center goals, mission, and vision.
    2. Cooperatively construct partnership agreements that maximize mutual benefits and minimize risks or place an undue burden on either partner.
    3. Monitor the health of current partnerships, proactively addressing deviations from partnership agreement, and highlighting mutual successes.
    4. In coordination with the Associate Director evaluate and process requests to utilize facility spaces. Collaboratively construct facility use agreements with Operations Team.Ensure a spirit of generosity of Esperanza towards agency or group utilizing facilities within parameters agreed upon.

EDUCATION, KNOWLEDGE, SKILLS, AND QUALIFICATIONS

  • Master’s degree in Public Health, Health Education, Community Health, Community Development, or related field required. Minimum 3-5 years of work experience in community/ healthcare settings required.
  • Knowledge of health care systems and related services, preferred
  • Knowledge of community resources, churches and community organizations, especially those in North Philadelphia (Fairhill, Hunting Park, and Kensington) preferred
  • Ability to effectively supervise, manage, and motivate staff.
  • Excellent oral and written communication skills.
  • Strong cross-cultural skills.
  • Demonstrated ability to handle many details and multiple priorities. Must show ability to organize, process, and verify details.
  • Demonstrate strong spiritual leadership and be able to integrate Christian faith with health care.
  • Adept with Microsoft Office Suite software and Google Suite.
  • Experience in health education curriculum development, preferred
  • Knowledge and experience with implementing and evaluating/measuring the effectiveness of public health initiatives.

WORKING CONDITIONS

  • Sit at a desk doing computer and telephone work for prolonged periods.
  • Lift 10-20 lbs.
  • Be able to work some nights and weekends.
  • Travel between EHC’s three clinic sites (Kensington, Fairhill, Hunting Park) as needed
  • Valid driver’s license required
  • Attend community events and meetings outside of EHC and outside of regular business hours, as needed
