Are you passionate about making a difference in the lives of chronically homeless individuals? We're seeking a dedicated Outreach & Rapid Response Coordinator to join our team. This role involves building relationships with individuals experiencing homelessness, connecting them to essential resources, and collaborating with various organizations to ensure their well-being.

Key Responsibilities:

  • Outreach and Engagement: Establish connections with chronically homeless individuals, particularly those who have recently been incarcerated.
  • Resource Coordination: Collaborate with shelters, outreach case managers, law enforcement, and community-based organizations to provide comprehensive support.
  • Crisis Response: Address immediate needs and provide referrals to mental health professionals for individuals experiencing emotional distress.
  • Community Partnerships: Foster relationships with disaster response entities and other relevant organizations.

Qualifications:

  • Minimum of 2 years of experience in active recovery
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team
  • Knowledge of mental health issues and resources
  • Proficiency in Microsoft Office and other relevant software
