Qualifications:
- Bachelor's degree in Social Work or related field
- Minimum BSW, MSW (preferred) or LCSW
- 1 year of Social Work Experience
- Skilled nursing experience preferred
- MDS experience preferred
Job Responsibilities:
- Work with residents, families and healthcare providers to help residents adjust to the Nursing Home setting in order to receive appropriate services to maximize independence.
- Plan, organize and lead individual or group conferences to understand, accept, and follow medical recommendations for residents.
- Provide recommendations to help restore resident to optimum well-being within resident’s capacity.
- Utilizes community based resources to assist residents to resume life in the community.
- Prepare service plan and promotes all rules regarding residents’ rights.
- Ability to work a flexible work schedule to accommodate for evening and weekend family appointments.
