Administrator / Director of Clinical Services (RN) - Hospice


Job Summary:

Constellation Health Services is seeking an Administrator / Director of Clinical Services - RN for our hospice program.

The Administrator/DCS assumes management responsibility for the fiscal and operational activities of the hospice organization. They are responsible for ensuring that patient care is coordinated and managed appropriately. The Administrator/DCS is responsible for ensuring that care and services are delivered appropriately and for providing supervision to the clinical personnel.  The Administrator/DCS oversees patient care services and personnel and makes patient and personnel assignments, and coordinates care and referrals. 


Qualifications:

  • Registered Nurse (RN) has the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state. 
  • Previous hospice and/or home care is required. 
  • 3-5 years' of health care management experience. 
  • Graduate of NLN accredited school of nursing with a BSN degree with 3 years of full-time clinical experience in nursing or a diploma of nursing or an associate degree in nursing and a minimum of three years of full-time or full-time equivalent clinical nursing experience in the last five years at least one year in a home health care agency or community health program which includes care of the sick at home.
  • Evidence of certification by the American Nurses Association as a community health nurse or completion of at least six credits in health care management from an accredited college or university program or school of nursing.
  • Complies with accepted professional standards and practice.
  • Demonstrates excellent observation, verbal and written communication skills.
  • Understands hospice philosophy and issues of death/dying.
  • Understands principles of pain/symptom management.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.


Responsibilities:

  • Directs the hospice and ensures implementation of policies and procedures regarding all activities and services provided by Constellation Hospice, whether provided through staff employed directly by the hospice, by volunteers or through contract arrangement.
  • Implements administrative and personnel policies.
  • Organizes and directs the ongoing functions of the agency.
  • Receives case referrals. Reviews available patient information related to the case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with the Medical Director regarding any questions about an individual’s eligibility for services.
  • Reviews the patient's medical diagnosis, prognosis, medications, procedures and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Assists the Director of Hospice Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long-range.
  • Complies with accepted professional standards and principles.
  • Performs other duties as requested.
