Duties & Responsibilities:
Clinical Support:
- Provides short-term case management services.
- Receives case assignments, establish plans, and review progress.
- Conducts regular meetings with assigned cases and community representatives.
- Offers individual counseling for motivational and psychosocial issues.
Outreach and Assistance:
- Conducts outreach and intake as needed.
- Provides counseling and assistance for self-sufficiency.
- Develops, implements, and monitors service plans.
Financial Assistance and Reporting:
- Screens for income supports and advocate for benefits.
- Provides financial assistance and support per organizational policies.
- Completes forms and reports accurately and on time.
Professional Development:
- Develops relationships with social service, health, and governmental agencies.
- Participates in meetings, training sessions, and conferences.
- Contributes to program and operational planning.
Qualifications:
- BA/BSW with three years of experience or Master’s degree with one year of experience.
- LPC/LSW license may be required.
- Valid driver's license may be needed.
Knowledge/Skills/Abilities (K/S/A):
- Fluency in a second language may be required.
- Ability to read, write reports, and speak effectively before groups.
Physical Demands:
- Stand, walk, sit, use hands, and reach frequently.
- Occasionally lift and/or move up to 25 pounds.
- Specific vision abilities required.
