Clinical Case Manager

The Clinical Case Manager will provide specialized clinical case management services to program participants, including intake, outreach, and case management. The ideal candidate will have experience working in a trauma-informed and/or mental health setting, with a strong understanding of Afghan culture and traditions. The Clinical Case Manager will work closely with a team of professionals to provide medical, psychological, and social work services to facilitate sustained physical, social, emotional, and economic well-being.
Key Responsibilities:
* Conducts intake, outreach, and provides clinical case management services to program participants
* Screens for income supports for benefits and entitlements to advocate on the behalf of program participants
* Receives case assignments, establishes service plans, reviews case progress and determines case closure through the supervisor of the assigned program and in accord with funder requirements
* May provide counseling to address motivational and psychosocial issues of program participants in an individual or group context
* Provides intense advocacy, crisis intervention, counseling and problem solving in a variety of areas to support, increase or restore program participant's capacity for self-sufficiency
* Develops, implements, oversees and follows up with program participant treatment plans to ensure that realistic goals and deadlines of accomplishments are being met within the reporting cycle outlined by program funders
* Develops, monitors and enhances professional relationships with social service, health and governmental agencies
* Participates and contributes in regular supervision meetings, team meetings, in house training sessions, conferences and program and operational planning
Requirements:
* AA/BA/BSW and three years experience working in a trauma-informed and/or mental health setting, or Master's degree in social work, counseling, clinical psychology or related field
* Fluency in Dari or Pashto strongly preferred
* Demonstrated commitment to anti-racism principles and the ability to apply an anti-racist lens to program design, implementation, and evaluation
* Demonstrated commitment to the principles of diversity, equity, and inclusion
* Demonstrated cultural humility, with a deep understanding of Afghan culture, traditions, and migration experience
