Position Summary:
The Outreach Specialist will conduct outreach activities to re-engage People Living With HIV (PLWH) into care and treatment services. The Outreach Specialist will skillfully strategize innovative ways to locate patients, and seamlessly transition clients to Specialized Medical Case Management (SMCM) which will use an individualized, strengths-based approach. Outreach Specialists and SMCMs will work as partners in re-engagement efforts. The main focus for the Outreach Specialist will be to ensure individuals who have fallen or are falling out of care are contacted and ultimately re-engaged into care and will spend a high percentage of time outside of the office conducting outreach activities. Outreach Specialists will also follow up with new clients who have missed their initial appointment(s) to ensure they are linked to care. Outreach Specialists will locate and interview clients to identify WHY the patient fell out of medical care and partner with SMCM to ensure patients are retained into medical care.
Essential Duties and Responsibilities:
The essential functions include, but are not limited to the following:
- Provides a wealth of knowledge to clients on what services are offered within their agency.
- Locates and contacts clients with a compassionate approach to ensure successful linkage and retention
- Interviews clients using the Return To Care Assessment (RTC) to identify barriers to continuous engagement in care and treatment
- Works closely in an inter-disciplinary team and with external organizations
- Participates in Care Conferences and Team Huddles with Medical Case Management (MCM) and clinic staff as clients miss a medical care appointment
- Informs clients how available services and medical care are beneficial
- Serves as part of the client’s support system (when needed) at the beginning of the SMCM intervention
- Communicates to supervisor service improvement suggestions that may prevent clients from falling out of care
- Manages a cohort of clients and systems to discreetly/contact clients
- Manages reports that are updated daily to review Outreach progress
- Maintains and researches client information and complete documentation in a professional and timely manner, in accordance with funder expectations
- Ensures that Protected Health Information (PHI) is kept confidential, in compliance with federal, state, and discipline-specific laws and regulations
- Able to work flexible work hours to support systems of contacting clients during alternate times
- Able to travel within the service area 40% or more each week
- Attends monthly Outreach Workforce Committee conducted by Ryan White (RW) Program in Columbia, SC
- Attend conference calls and webinars as offered, to optimize results and ensure coordination of service and effort
- Continual CQI
- Conduct HIV/AIDS screenings when needed, not limited to testing in-house or outside agencies.
- Make appropriate referrals for clients to obtain adequate and relevant services as identified in their assessments.
- Enrolls and recertifies clients in SC ADAP (AIDS Drugs Assistance Program) as needed
Note: Outreach Specialist may be cross-trained to conduct rapid HIV testing to support RW Part B - Early Identification of Individuals with HIV/AIDS (EIIHA) activities (i.e., Test Partners of clients as clients return to care).
Minimum Qualifications/Education/Skills and Abilities:
Undergraduate Degree with compassionate Outreach experience
- Possesses valid driver’s license, as position will require driving duties
- Conducts activities with vigor, compassion, and team-oriented approach
- Uses good communication skills, creativity, flexibility, organizational skills
- Maintains knowledge of community resources and familiarity with the service area
- Researches available sources to find valid client contact information
- Develops and maintains a positive rapport with clients, co-workers, and community stakeholders
- Interacts with a diverse population of clients while performing outreach activities
- Maintains a non-judgmental approach while interacting with clients and co-workers
- Demonstrates commitment to long term success
- Applies initiative to handle various tasks simultaneously
- Develops rapport while interacting with clients over the phone, during field visits and using alternate delivery channels (in accordance to agency policy)
- Sets priorities and keeps supervisor informed of work progress, time tables and outcomes
- Meets tiered productivity and success goals as established in the Outreach Workforce Committee forum
Physical Requirements: Must be able to travel both in state and to occasional national meetings. Must have direct client contact in order to provide program activities. Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
