Position Description

The Healthcare Case Manager is responsible for providing supportive housing services by delivering medical and mental health case management to clients with HIV/AIDS. The Case Manager is also charged to provide HIV education and entitlement advocacy services to clients. S/He will be responsible for all compliance standards of HASA.

Essential Job Functions

The following duties are mandatory requirements of the job:

  1. Conduct in-person and/or virtual monthly home visits according to Scattered Site Desk Guide and Harlem United policies.
  2. At the monthly home visits conduct apartment inspections and submit maintenance request for repairs that requires Harlem United maintenance work as well as for repairs that require Landlord interventions.
  3. Monitor the client’s progress through scheduled office visits, home visits, and phone contact as needed as well as anticipating and dealing with crisis situations that may also include maintenance issues and/or the deterioration or loss of an apartment.
  4. Assist the client in identification of daily life challenges while providing timely and vigorous documentation of services. Maintain compliance with all HASA requirements, Desk Guides, and Protocols and Standards.
  5. Complete reassessment and treatment plans that involve counseling, goal setting, and identification and coordination of supportive services.
  6. Participates in weekly administrative and clinical supervision and is responsible for developing skills and increasing knowledge as provided by supervisor, training, or other educational tools and services.
  7. Complete progress notes to corroborate contacts, advocacy, office visits and monthly Home visits
  8. Completing annual documentations (e.g. HIPAA, Housing forms, Terms & Conditions)
  9. Collaborating with the Clinical Director or Mental health providers (internal/external) around obtaining/completing initial/annual psychosocials
  10. Provide client escorts as needed

The following duties are to be performed as assigned by the supervisor:

  1. Deliver advocacy to client healthcare by providing an affirmative and assertive approach to assisting the client with services that they are entitled to.
  2. Recognize how the external demands of the client’s environment may impact their medical, mental health and psychosocial needs.
  3. Aid the client with skill development that will lead to a more independent level of functioning in daily life.
  4. Identify and remove barriers to a client’s medical and mental health care while linking the client to supportive services within Harlem United or outside agencies as needed.
  5. Consult with Residence Management and/or Maintenance Coordinator regarding the scheduling and delivery or maintenance requests for clients’ apartments.
  6. Participate in case conferences with all agencies that have been or are currently involved in the clients’ care. Obtain copies of case records as necessary.

Education and Certification

BA degree and 3 years of experience in direct case management with 2 years in HIV/AIDS treatment and a working knowledge of substance use issues and interventions. Solid oral and written communication skills, good time management, and good interpersonal skills are required. An interest in professional growth within a health-driven social service agency is encouraged. Additional certificates or trainings a plus.

  1. Basic computer literacy (e.g., Microsoft Word, Outlook, Excel)
  2. Excellent case management skills
  3. Experience working with people living with HIV/AIDS
  4. Superior written and verbal communication skills
  5. Leadership and team-building skills
  6. In an emergency, willing and able to provide services outside of normal business hours
  7. Bilingual (Spanish) a plus
