The Case Manager identifies and implements social services for homeless families, to ensure successful transition into permanent housing as soon as possible. Reports to the Director of Social Services.

**ESSENTIAL FUNCTIONS**:
**Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions for case manager.**
- Ensures that clients understand and work towards meeting their responsibilities in securing permanent housing as soon as possible
- Establishes and maintain ongoing client case files
- Maintains current and accurate progress notes
- Ensures information in client file is accurate and current, i.e., family composition, etc.
- Documents all information, referrals and action taken in case file
- Assists families in obtaining the required documentation to obtain permanent housing.
- Ensures that all supporting documents are printed and filed in designated sections in case record.
- Meets with clients bi-weekly to determine what the family’s needs are, to accomplish the goal of moving into permanent housing as soon as possible
- Assists clients in identifying the need for specific services and develop a mutually acceptable Service Plan
- Analyzes case findings to determine if needs have been addressed through referrals and current service plan
- Identifies community based organizations, agencies and as potential resources for linkages.
- Attends Building Meetings, agency-wide meetings, staff meetings and training.

**QUALIFICATIONS**:

- BA/BS Degree in Social Work or related field.
- Three years casework experience working with the homeless
- Excellent written and oral communication skills
- Knowledge of community resources and linkage
- Highly motivated, well organized, and flexible
- Must be computer literate

Pay: $29.09 per hour

Expected hours: 35 per week
