Job Title: Admissions Coordinator


Job Summary:

We are seeking an Admissions Coordinator to join our team at Santa Marta. The Admissions Coordinator will be responsible for coordinating admissions from both internal and external sources to our Health Care Center. The successful candidate will be responsible for coordinating initial assessments, meeting with Santa Marta residents and family members to discuss transfer to Health Care Center, and creating and maintaining an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department.


Responsibilities:

- Coordinate admissions from both internal and external sources to Santa Marta Health Care Center

- Coordinate initial assessments for new residents

- Meet with Santa Marta residents and family members to discuss transfer to Health Care Center

- Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department

- Maintain accurate and complete records of all admissions and assessments

- Collaborate with other departments to ensure a smooth transition for new residents

- Provide excellent customer service to residents and their families

- Ensure compliance with all state and federal regulations related to admissions and assessments
