POSITION FUNCTION SUMMARY

Under the general direction of the Senior Director of Nursing (SDON), the RN Care Coordinator Manager is responsible for rendering a variety of skilled nursing care in assigned areas in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of nursing. The Care Coordinator Manager is responsible for the supervision and leadership of all Care Coordination staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES

This position may have various work assignments within HICHC. This description is intended to be generic in nature, and as such it does not detail all duties and responsibilities of the job assignment. Various duties, responsibilities and accountabilities may be assigned to an incumbent in this position depending on clinic needs, and may include but not be limited to the following:

Supervisory Duties

  • Works with the Senior Director of Nursing, Medical Director, medical providers and support staff to ensure there is an integrated healthcare delivery system at HICHC that is efficient, effective and timely with appropriate response to patient and community needs.
  • In consultation with Senior Director of Nursing and Medical Director, plans and oversees health service delivery by care coordination staff, makes staffing assignments and supervises and evaluates the quality of care delivered.
  • In coordination with the Quality Improvement team provides leadership to care coordination staff for continuous quality improvement efforts in primary care delivery and documentation. Implements and/or updates processes for data collection, tracking and documenting activities.
  • Provides leadership to ensure the highest standards of excellence in customer service by anticipating and responding to needs of customers and internal partners; proactively works to improve the quality of customer service provided by HICHC.
  • Leads personnel in such a manner as to promote a productive, efficient, and safe working environment; establishes an effective flow of communication and information to ensure that problems can be resolved in a timely, effective manner and that support is provided to staff as needed.
  • In coordination with the Human Resources Manager and SDON, is responsible for the selection, assignment, training, and development of assigned direct report personnel.
  • Ensures implementation of an effective performance management process for all direct report staff that clearly defines performance expectations and measures, including goal and objective setting, performance feedback, accountability measures, and corrective action as appropriate, and in compliance with sound management practices and applicable laws and regulations to ensure high levels of organizational, work unit, and individual performance.
  • Actively seeks continuing education and training tools to support best practices for care coordination staff and shares information through staff in-service training and individual performance improvement training. Conducts and documents regular care coordination staff meetings.
  • Manages care coordination inventory to ensure that necessary supplies are ordered and available, are properly stored and maintained, while seeking ways to reduce waste of resources and supplies.
  • Maintains compliance with State licensure by enforcing Quality Assurance protocols are adhered to in all patient care areas.
  • Ensures regulatory compliance of HIPAA by monitoring the workplace environment, on- and offsite staff activities and patient care services provided. Identifies areas where improvement or training is needed.
  • Handles patient complaints and/or questions in care coordination areas and addresses appropriately or forwards to SDON expeditiously.
  • Develops, reviews and updates policies, standard operating procedures and documentation tools as required in collaboration with the SDON, Medical Director and QI team.
  • Ensures timesheets and PTO requests are entered in ADP, monitored and approved in a timely manner in coordination with the payroll department.
  • Attends budget and grant-related meetings, completes grant reports as required and works with leadership and the finance department to appropriately utilize grant funds and resources.
  • Completes other projects and duties as assigned by SDON, Medical Director, COO, and CEO.

Clinical Nursing Duties

  • Follows Nursing Practice Standards to evaluate patient needs and formulate and implement an appropriate nursing care plan to include physiological, psychosocial, and environmental factors. Identifies the patient’s formal and informal support systems. Revises plan of care based on ongoing assessments as needed with any new data or changes in patient condition or support systems.
  • Identifies a cost-effective comprehensive care plan to meet patient and family needs and ensures services are delivered in collaboration with medical and behavioral health services.
  • Documents nursing assessment and patient care provided, including nursing intervention, patient response to care provided, patient needs, problems, capabilities, limitations, and progress towards goals. Documentation includes evidence of appropriate patient, family, and significant other teaching, and the understanding of these instructions is noted in record.
  • Communicates with the medical provider regarding patient needs, nursing assessments, interventions and patient education in a timely manner.
  • Acts as a patient advocate. Provides referrals to appropriate community resources; facilitates access and communication when multiple services are involved; monitors activities to ensure that services are being delivered and meet the needs of the client; coordinates services to avoid duplication. Works with HICHC referral specialist to provide appropriate services.
  • Educates patients by disseminating patient information and literature relating to health education programs including but not limited to discharge planning, family planning, pre-natal care, and health promotion/disease prevention for all age categories. Conducts periodic patient evaluations and knowledge updates as appropriate.
  • Performs direct patient care, utilizing established procedures, policies, and standards as evidenced by client/staff feedback and physician observation.
    1. Assesses patient, collects and documents the patient’s current health status and medical history. Makes accurate initial and ongoing nursing assessments and responds appropriately. This may occur in person at scheduled appointments, walk-in visits and via telephone calls.
    2. Demonstrates ability and appropriate technical skills when administering medications, IV’s, medical/nursing treatments and nursing procedures in accordance with medical provider's orders, standing operating procedures and HICHC policies. Assesses and documents response to treatment and compliance with clinical regimens. o Demonstrates necessary skills and knowledge to provide care for patients of all ages according to policy and procedure and primary care nursing competencies.
    3. Demonstrates ability to handle emergencies in a prompt, precise, professional manner per nursing training and HICHC protocol.
    4. Performs personal patient care in a manner to provide comfort and well-being to the patient, acknowledging physiological and psychological needs.
  • Provides support for clinical activities by acknowledging, adhering to, and implementing organizational, nursing and medical policies and standard operating procedures.
  • Performs clinical case management as needed by contacting, teaching, and following protocol for patients with abnormal laboratory or imaging results or by providing assistance with maintaining sufficient medication or medical supplies.
  • Maintains patient files by updating information pertaining to medications refills, laboratory results, patient medical visit and other patient data required by medical providers.
  • Maintains disease management charts and flowsheets related to the case-management track system and monitoring of high-risk populations.

Education and Experience

This position requires a bachelor’s degree in nursing, and a minimum of 1 year of supervisory nursing experience (charge nurse experience is applicable); 5 years progressive experience in a community health center and working within a clinical health care setting preferred. In lieu of a bachelor’s degree, the incumbent must have an associate degree in nursing and possess sufficient prior work experience in progressively responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.

Licenses and Certifications

  • Current Nursing license for the State of Hawaiʻi is required, and documentation of education, experience and licensure is required and must be maintained; incumbent must be insurable under the policies and requirements of HICHC.
  • Current BLS certification is required and must be maintained.

Knowledge, Skills and Abilities

The Care Coordination Manager must be familiar with the community health care field and must have a sound understanding of and ability to apply leadership and management principles and methods. Additional knowledge and skills important to this position include but are not limited to:

  • Demonstrates knowledge of the principles of growth and development over the life span and incorporate them in their plan of care and health education.
  • This position requires advanced knowledge of clinical patient intake procedures, and laboratory and pharmacy protocols.
  • Supervisory skills and knowledge of clinical management techniques regarding various clinical functions (i.e. JCAHO, OSHA, lab, pharmacy, infection control) and familiarity with state and federal laws and regulations f or the operation of nursing services is essential.
  • Interpersonal relations and customer service skills; facilitating interdepartmental operations.
  • Knowledge of the applicable administrative standards, rules and regulations
  • Good organizational and self-management skills, including time management, document handling and management, etc.
  • Must be able to work on multiple tasks within established deadlines.
  • Must be able to work independently under the general direction of a provider and follow instructions for work completion.
  • Working knowledge of computer systems including but not limited to word processing, spreadsheet, database, and electronic communication software applications.

Language Skills

  • Ability to read and interpret documents such as patient records, operating and maintenance instructions, procedure manuals, etc.
  • Ability to write at an above average level of competence, including internal reports and memoranda.
  • Ability to communicate with diverse groups of people to include staff and providers and patients.
  • Ability to communicate effectively with patients and their families to make their visit a pleasant experience.
  • Sensitivity to the multicultural nature of the service area population and may be required to communicate in another language.
