ESSENTIAL DUTIES & RESPONSIBILITIES

  • Hire, supervise, evaluate, schedule, terminate and direct the work of department personnel.
  • Oversee and assist with Wellness Clinic and Home Care operations.
  • Comply with and adhere to state regulations on the licensing of Home Care Agencies. Establish and maintain necessary records for compliance.
  • Lead department in the continual review of residents, service planning based on that review and changes in care levels.
  • Coordinate health services with residents, physicians, therapy, recreation services and spiritual life for optimal care.
  • Coordinate visits and serve as main contact and liaison for CHS clinic, podiatrist, dental clinic and geriatric psychiatrist.
  • Maintain updated Policy and Procedure Manual for department.
  • Prepare and work within facility's approved department budgetary guidelines and operational plan.
  • Collaborate with Recreation Services to deliver wellness and fitness activities, as well as coaching services, to residents, all in conjunction with interdisciplinary team.
  • Work closely with community resources to develop and maintain year-round wellness initiatives.
  • Evaluate the quality and effectiveness of the program and services to determine compliance. Create action plans for improvement if indicated.
  • Participate as a member of various committees. (QA)
  • Maintain positive working relationship with residents, employees, other departments, management and representatives of external groups.
  • Comply with company policies and procedures, related local, state and federal regulations and with accepted professional standards and practices.
  • Adhere to the defined policy on the Bill of Rights for nursing home residents.
  • Remain current on Nursing techniques and advances, as well as latest trends in long term care by attending in-services, seminars, and continuing education programs.
  • Perform other duties as assigned.

EDUCATION/ EXPERIENCE

Must be a graduate of an accredited school of nursing, and possess, as a minimum, the earned title of Registered Nurse.

Two to three years long term care experience as a registered nurse to include one year of management and/or educational training experience
