Job Description: As a Care Coordinator, you will play a crucial role in managing and coordinating the care of our clients. You will work closely with clients, families, and healthcare providers to ensure that all aspects of care are delivered seamlessly and efficiently. Your primary responsibility will be to create and oversee care plans that meet the unique needs of each individual, ensuring that they receive the highest quality of care possible.

Key Responsibilities:


  • Develop, implement, and monitor individualized care plans for clients.
  • Act as a primary point of contact for clients, families, and healthcare professionals.
  • Coordinate and schedule medical appointments, treatments, and therapies.
  • Advocate for clients by ensuring their needs and preferences are communicated and addressed.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as necessary.
  • Maintain accurate and up-to-date records of client care and interactions.
  • Provide support and guidance to clients and their families, addressing any concerns or questions.
  • Collaborate with healthcare providers and community resources to ensure comprehensive care.
  • Stay informed about the latest industry trends and best practices in care coordination.
Qualifications:


  • Bachelors degree in Nursing, Social Work, Health Administration, or a related field (preferred).
  • Certification or licensure in relevant field (e.g., RN, LCSW) is a plus.
  • Proven experience in a care coordination or case management role.
  • Strong understanding of healthcare systems and services.
  • Excellent organizational and multitasking skills.
  • Exceptional communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate, empathetic, and dedicated to providing outstanding client care.
