Job Summary

We're seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you'll play a critical role in ensuring the safety and well-being of our clients. You'll work closely with our clinical team to provide supervision, support, and guidance to our clients. This is a dynamic and rewarding role that requires a strong understanding of behavioral health principles and a passion for delivering exceptional care.

Responsibilities
  • Provide supervision and support to clients, ensuring their safety and well-being at all times.
  • Work closely with the clinical team to develop and implement individualized care plans.
  • Facilitate daily schedules, including activities, groups, and events.
  • Provide transportation to clients for community outings and meetings.
  • Maintain accurate records and documentation, adhering to all policies and procedures.
Qualifications
  • H.S. Degree or GED required; bachelors level degree in psychology, counseling or sociology preferred.
  • Prior work experience in behavioral health treatment settings is a plus.
  • Valid driver's license and eligibility for insurance coverage required.

Physical Demands

This role requires the ability to walk, sit, stand, bend, and lift, as well as the mobility to access the house/campus. The incumbent must be constantly alert for client safety.
