Job DescriptionJob Description

Signallamp Health, Inc is currently seeking a Care Management Enrollment Specialist in the Eastern Time Zone.

The patient engagement specialist enrolls patients in a new care management program for patients with multiple chronic conditions.

Your primary duties include but are not limited to:

- making outbound calls to patients who are eligible to participate in their provider's care management program.

- describing the benefits of the program and the reasons their primary care physician recommends enrolling

- encouraging patients to enroll in the program

- gathering and recording certain patient information in accordance with policies and procedures

- using call scripts to communicate all necessary information to patient

- meeting productivity metrics (with incentive bonus if minimum metrics exceeded)

Remote work available following training and probationary period

Experience with Electronic Medical Records a plus
