Outreach Specialist


35 hours per week / $24.00 - $25.00 per hour


(The actual compensation will be determined based on experience and other factors permitted by law.)

Summary: Under the direct supervision of the Program Manager, provide outreach, engagement, and linkage to services for individuals experiencing homelessness in the City of Long Beach. Work collaboratively as a part of a team providing services to individuals experiencing homelessness with possible mental health, substance use, and physical health conditions based on psychosocial rehabilitation model of treatment and services.

  • Coordinate and provide outreach services for individuals experiencing homelessness in the City of Long Beach
  • Assist individuals in accessing resources for basics needs (food, clothing, shelter)
  • Work collaboratively as part of an integrated team to provide outreach and linkage to community-based case management, housing resources, mental health and primary care services to individuals experiencing homelessness
  • Assist in devising and implementing strategies for engaging hard-to-serve individuals who are unhoused
  • Make independent assessments to identify individual needs as necessary and initiate communication and collaboration within the integrated team and with referral sources to provide the resources to meet those needs
  • Utilize evidence based practices in service delivery such as motivational interviewing, harm reduction, housing first, supported employment and education, and intensive case management
  • Assist individuals to gain or increase income from general relief, social security, disability, or employment
  • Assist individuals to access existing transportation resources
  • Assist individuals to obtain necessary documents as needed/required for housing resources (ID, social security card, birth certificate)
  • Provide health education, resources, and connection to community health providers
  • Provide access to harm reduction supplies/services and refer/link participants to community substance abuse treatment resources as appropriate (i.e., detox/medical care, outpatient treatment, sober living/residential programs, counseling)
  • Build collaborative partnerships with existing and new community resources and providers for linkage and support purposes.
  • Complete all documentation and maintain all paperwork and other required documentation as required for program compliance and accountability in accordance with MHALA standards and funding source requirements.
  • Participate in staff and community meetings and the day-to-day operations of the program as needed

Position Requirements:

  • High school diploma or GED with minimum one year of homeless services or related experience required
  • Bachelor degree in a human service or related field preferred
  • Must have good basic English writing skills, good basic computer skills and be highly organized. Must have excellent communication skills.
  • Proficient in computer applications such as Microsoft Word/Office, Excel, and Outlook
  • Must be able to have the ability to juggle multiple tasks without losing productivity.
  • Must function well in a team-oriented approach to member services; demonstrate knowledge of community resources
  • Must possess a high level of tolerance and understanding for a work atmosphere that is very busy and highly unstructured and for individuals who present for services with urgent multiple needs, issues and problematic behavioral characteristics
  • Must know or very quickly become knowledgeable of the characteristics of individuals who are substance abusing and/or dually diagnosed, and have knowledge of local services available for dually diagnosed individuals
  • Flexible work schedule to include some evenings or weekends
  • Driving with current class C driver’s license, a vehicle for transportation, proof of valid automobile insurance, the ability to be insured by MHALA liability insurance are conditions of employment
  • CPR certification
  • TB clearance and re-testing as required are conditions of employment
