Your day-to-day as a Case Manager-Men's SAAs a Full-Time Case Manager for Men's Substance Abuse at Hampton-Newport News Community Services Board, you will play a vital role in ensuring continuity of care for individuals facing complex challenges related to mental illness, intellectual developmental disabilities, and substance abuse. Your responsibilities will encompass various essential services, including outreach, psychological assessments, treatment planning, and advocating for clients' needs. You'll be directly involved in developing tailored service plans while coordinating various resources necessary for each individual's well-being. This dynamic position requires adept interaction with multiple professional disciplines and offers opportunities for crisis intervention, supportive counseling, and daily living support. You will be called upon to demonstrate creativity and innovation, particularly in high-stress situations involving hostile or aggressive individuals, where de-escalation skills are paramount.
Your expertise will empower clients to overcome barriers, address their immediate and long-term needs, and ultimately foster recovery and resilience within the community.
What matters mostTo excel as a Full-Time Case Manager for Men's Substance Abuse at Hampton-Newport News Community Services Board, candidates must possess a Bachelor's degree in Human Services, such as in Social Work, Psychology, Sociology, or Counseling-along with at least one (1) year of experience providing case management services to individuals living with a diagnosis of substance use disorder or co-occurring diagnosis of substance use disorder and mental illness. To effectively interact with diverse populations, proficiency in assessment and treatment planning is essential, along with excellent communication and interpersonal skills. Strong crisis intervention skills and the ability to de-escalate potentially volatile situations are crucial. A valid Driver's License is required, as the role includes transporting individuals as needed. The ideal candidate will demonstrate adaptability, problem-solving, and creativity to address resource limitations while fulfilling individual needs.

The knowledge and skills required for the position are:
