Requirements:- Active RN license in the of RI
- Completion of comprehensive assessments and associated individualized care plans
- Participates in outreach/marketing activities as needed and appropriate
- Maintains and up-to-date knowledge of resources in the state of Rhode Island
- Capable of providing case management services in an efficient, effective, and collaborative manner to help avoid duplication of services, costs and administrative tasks.
- Responsive to requests for information and/or assistance from individuals in a timely manner
- Respectful of individual's rights, strengths, values and preferences; encouraging the individuals to create, direct and participate in their person-centered plan to the fullest extent possible.
