Summary of Responsibilities:

Under the general direction of the IL & Family Finding Supervisor, this position functions in a relatively independent manner with the incumbent expected to know what their duties/responsibilities are, complete them, and keep the Supervisor apprised of program flow. Duties are conducted in accordance with all existing policies, procedures, and regulations and in accord with the Mission and Values of the Company.

Essential Functions:

  • Assists with/initiates referrals.
  • Helps establish youth stability in apartments.
  • Prepares service plans
  • Work in individual/group settings.
  • Prepares court documents.
  • Attends hearings.
  • Coordinates meetings.
  • Assists with transportation needs.
  • Schedules monitoring visits.
  • Assist youth in transition to independence.
  • Teaches appropriate social skills and interaction.
  • Teaches basic culinary, homemaker, management, and maintenance skills.
  • Incorporates PREP program.
  • Conducts assessments.
  • Assists with obtaining photo identification, birth certificates, and social security cards.
  • Assists with self-esteem groups.
  • Provides supportive feedback and engagement.
  • Completes case notes.
  • Attends retreats and conferences.
  • Hosts local groups.
  • Coordinates educational groups and professional speakers.
  • Assists with data collection and input.
  • Enters information into data bases.
  • Maintain and submit supporting documentation as required by grant guidelines.
  • Prepares reports, maintenance requests, and relevant letters.
  • Assists with requisitions.
  • Types correspondences.
  • Attends meetings.

Education and Experience:

Bachelor's Degree supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.

Employment Requirements:

Automobile and Valid Insurance, Valid Pennsylvania Driver's License, acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance

Working Conditions:

Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, and travel throughout the assigned region may occur and may be in adverse weather conditions, must be able to lift to 15 pounds
