Currently, we are seeking a self-driven Clinical Coordinator to join our team in our San Francisco Office. As we approach our 20-year anniversary of providing high-quality care to our patients, we are looking for someone who shares our passion for improving healthcare outcomes and patient experience.


As an essential member of our team, you will have the opportunity to work in a collaborative and supportive environment with our clinical and office staff in San Francisco. This is an IMMEDIATE job opening for our San Francisco office, the ideal candidate is a dynamic, high-energy, team player ready to join our team and hit the ground running

We are looking for someone that is willing to demonstrate exemplary performance in their ability to prioritize, multitask, organize, and learn quickly. We are looking for a detail oriented individual, with an extremely pleasant demeanor who possesses excellent communication skills and is comfortable with handling a large call volume As a Clinical Coordinator, you will ensure and coordinate patient care along with working collaboratively with our exceptional team of healthcare professionals.

What You'll Do:

  • Clinical Coordinator will support field clinicians in providing excellent patient care
  • Clinical Coordinator will assist the clinical team in requesting, clarifying, faxing, and tracking of orders.
  • Input orders, float visits, and obtain supporting documentation as necessary.
  • Communicate with patients, families and physicians regarding their plan of care.
  • Upload orders into the electronic medical records as appropriate
  • Assist in the development and coordination of orientation of new organization personnel.
  • Order and monitor medical supplies to ensure adequate stock available.
  • Maintain organization of the medical supply room per company standards and OSHA requirements.
  • Complete audits as assigned.
  • Perform other duties as assigned.

How You'll Be Successful:

  • You have a current California RN or LVN License
  • You have experience working in Home Health
  • You demonstrate strong verbal, written and interpersonal communication skills.
  • You have familiarity with company-provided Apple products
  • You demonstrate time management, organizational skills, and ability to function with minimal direction. You are a detail oriented individual with excellent communication skills
  • Extremely pleasant on the Phone & comfortable with large call volumes
  • Knowledge of medical terminology
  • Demonstrates strong verbal, written and interpersonal communication skills
  • Strong computer skills
  • Demonstrates time management, organizational skills, and ability to function with minimal direction

Requirements:
  • Associate (Preferred)
  • RN or LVN License or Degree
  • Attention to detail and accuracy in scheduling tasks
  • Fast learner with the ability to adapt quickly to new systems and processes
  • Strong organizational skills to manage multiple schedules efficiently
  • Excellent communication skills, both verbal and written
  • Ability to work collaboratively in a team environment
  • Proficient computer skills, including knowledge of scheduling software
  • Understanding of healthcare regulations and compliance requirements
  • Customer service-oriented mindset with a focus on patient satisfaction

Experience:

  • Nursing: 1 year (Preferred)
  • License/Certification:
  • BLS Certification (Preferred)
  • RN or LVN License (Required)
