This position is responsible for assisting the department director with overseeing occupational therapy programs for clients with compromised health conditions. Provides education, coordinates services, and administers occupation therapy in accordance with defined treatment programs for fall prevention including neuromuscular education, therapeutic activity, exercise, manual therapy, self-care/home management training, development of cognitive skills, sensory integration techniques, and/or wheelchair management.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

1. Assist the Director of Home Safety & Accessibility with the development of a comprehensive home assessment tool to identify individual needs and to promote fall prevention.

2. Complete home assessment for referred individuals and work to assist them with the creation of a person-centered fall prevention plan, including referrals, for durable medical equipment and home modifications.

3. Develop scope of work for projects and oversee work with contractors and medical equipment distributors to coordinate install/delivery of needed modifications and equipment.

4. Perform occupational therapy services in accordance with client needs.

5. Assist with providing instruction and education on use of supplied equipment and follow up with individuals served to answer questions as needed.

6. Coordinate with other community resources to build relationships for referrals as appropriate.

7. Assist with the development of fall prevention education and materials for individuals, caregivers and staff.

8. Maintain Certified Aging in Place (CAPS) and other certifications needed to successfully complete home assessments in the community.

9. Assist the director with the implementation of new programs and grants to coordinate fall prevention programs and services outside of Marion county.

10. Communicate with home modification specialists and care managers as needed to ensure all needs identified during home assessment are being met for case managed clients.

11. Travel locally as needed.

12. Perform other duties as assigned.

Education and/or Experience Bachelor’s in occupational therapy or related field with valid licensure to complete occupational therapy. Minimum of 1-2 years’ experience in home health care with the elderly or disabled. Ability to manage clients varying needs; excellent knowledge of principles/practices of occupational therapy; excellent interpersonal communication skills; familiarity with ADL evaluation tools preferred
