Job description

Absolute Hospice is looking for a Full-Time RN Case Manager for its team. This position is a great opportunity for the right candidate If you are a motivated, focused, RN looking for an opportunity to make an impact, this is an opportunity for you.

Responsibilities:

  • Triaging calls
  • Ensuring medical orders are transcribed and processed accurately
  • Competence in evaluation of patient needs
  • Promoting continuous quality improvement
  • Teaching and counseling patients/families
  • Timely and accurate documentation
  • Case Managing
  • Other duties as required

Requirements

  • Graduate of an approved nursing program/school is required. Bachelor's Degree, preferred.
  • At least 1 year of recent acute care experience in an institutional setting is required. At least 1 year of recent experience in Home Care is required.
  • At least 1 year of Hospice experience is preferred.
  • Active Michigan RN License is required.
  • Active AHA BLS Certification is required.
  • Responsible for supervising Hospice aides, supervisory/management experience is preferred but not required.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
  • Must possess a valid driver's license and acceptable driving record

Job Type: Full-time

Schedule:


  • Holidays
  • Monday to Friday
  • On call
  • Weekends as needed


Education:


  • Bachelor's (Preferred)


Experience:


  • home care/hospice: 1 year (Preferred)


License/Certification:


  • RN (Required)
  • CPR Certification (Preferred)
