Job DescriptionJob Description

Primary Location:

  • Provides Health Home approved services at any OCO portal or satellite site; the offices of a Community Partner, in the community, home or any other venue as applicable

Consumers:

  • Works with those with a variety of medical conditions and victims of domestic violence, sexual assault, child abuse; crime; perpetrators; sex offenders; homeless; those with substance abuse issues; mental health issues; learning and developmental disabilities
  • Is responsible for the case files of multiple consumers.
  • Responds to consumers who are contacting in person and on the phone to provide information about services and the status of their services,
  • Meets with the consumer, primary caregiver/advocate/guardian and families involved in the Health Homes service system and prepares them for the various stages of the system in which they are involved.
  • Commonly assists consumers involved with the child protective, social service and/or legal systems through advocacy, accompaniment and general support.
  • Works individually and as part of a team to provide home visits and provides case management services, connections to community supports and follow-up.
  • Advocates, accompanies and/or negotiates on behalf of consumer and/or their advocate/guardian.
  • Conducts Assessments for services
  • Carries out duties as assigned with regard to Health Home Criteria

Responsible for:

  • Tracking and compiling statistics related to program activities and outcomes
  • Having working knowledge of Medicaid Billing and Health Home documentation requirements, protocols and deadlines including entry into MAPP and other mandated databases.
  • Providing and arranging adequate, up to date training for community partners
  • Staying up to date with best practices in the field
  • Being on call on a rotating basis to address emergency needs of consumers

Other:

  • Develops and maintains effective working relationships with community partners, such as social service, child protective service, criminal, civil, and law enforcement agencies.
  • Must maintain good standing with the Justice Center as well as pass a criminal background check
  • Must maintain extensive and detailed documentation of consumer files or consumer contacts, advocacy and any other relevant casework information by deadline on the MAPP Database
  • Must be proficient with crisis management, de-escalation techniques and problem solving and health conditions
  • Carries out other special projects as assigned.
  • Must possess a Bachelor’s degree from an accredited school in: child & family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation, recreational therapy, rehabilitation, social work, sociology, or speech and hearing; OR NYS licensure and current registration as a Registered Nurse and a bachelor’s degree; OR a Bachelor’s level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR a Credentialed Alcoholism and Substance Abuse Counselor (CASAC).

Full-time - 37.5 hours per week
