LPN - Float Clinic

Reports To- Physician, Clinical Lead, and Clinic Director
1.Provides for patient and family safety, maintains an environment to promote health.
2.Provides for psychosocial, ethical and religious needs of patients, families and health care workers, and intervenes creatively to meet individual needs.
3.Assist patients with activities of daily living maintains comfort and privacy.
4.Participates in all aspects of patient care.
5.Is competent in making accurate assessments of patient's condition, complaint and needs. Gathering and clustering of data. Applying critical thinking skills toward making nursing judgments, communicating relevant findings to the physician, and confidently and efficiently carrying out the plan of care.
6.Exhibits understanding of laboratory policy and procedures and is proficient in obtaining and testing laboratory specimens.
7.Safely administers medications with regard to "Five Rights" rule and in accordance to clinic policy and procedure.
8.Dependable in completing physician orders, treatments, and assisting physician with procedures in accordance with policy and procedure.
9.Works with the physician to emphasize routine health screening, promote preventative medicine and provide patient education.
10.Collaborates with the supervising physician to ensure all clinic duties are accomplished, quality patient care is given, and progress and follow-up is monitored.
11.Understands and demonstrates the proper use and maintenance of supplies and equipment.
12.Aides in identifying clinic problems and participates in activities directed toward change.
13.Demonstrates and implements a working knowledge of policies and procedures and of the intrdisciplinary process.
14.Demonstrates interpersonal skills in working with licensed and unlicensed personnel Supervises duties of Certified Nursing Assistant.
15.Participtes in the orientation and training of any newly hired licensed practical nurse or Certified Nursing Assistant.
16.Maintains competency of skills.
17.Carries out other appropriate duties assigned by the Physician, Clinical Lead, or Clinic Director.
Acts within the realms of a current Mississippi Licensure and under the direct supervision of the physician.
Purpose- The fundamental responsibility of the nurse is fourfold:
1.To promote health
2.To prevent illness
3.To restore health
4.To alleviate suffering
The nurse can only up-hold these responsibilities by working as one with the physician. The nurse should be instrumental in enabling the physician and this facility to provide quality care by:
1.Assessing situations.
2.Anticipating and meeting needs.
3.Monitoring and communicating information effectively.
4.Managing resources and time efficiently.
5.Giving quality nursing care.
Specific duties routinely expected from the L.P.N.
1.Obtains patient history information.
2.Records all current medications and treatments.
3.Assesses patient's condition, complaints, and needs.
4.Collects diagnostic reports, communications from other healthcare providers, insights from family members or caregiver, E.R records, old charts and other pertinent information.
5.Makes nursing judgments based on interpretation of specific information obtained from patient history, awareness of acute and chronic health factors, observations, evaluations of symptoms, precipitating factors and response to relief measures.
6.Reviews the response, progress, or outcome of current and previous treatments.
7.After collecting and validating subjective and objective data and interpreting this data the nurse organizes the information into meaningful clusters.
8.Initiates Nursing interventions.
9.Prepares and presents the patient, along with a summary of this information to the physician for examination, assessment, diagnosing and treatment orders.
10. Carries out all treatment orders according to clinic policy and procedure. Performs treatments in a safe and efficient manner.
11.Administers ordered medications in accordance with the 'Five Rights" rule. Observes Patients Rights related to drug administration.
12.Procedures:
(A) Reviews patient's understanding of and answers any questions concerning the ordered procedure.
(B)Obtains signature of informed consent for ordered procedure.
(C)Prepares patients for procedure.
(D)Sets-up for procedure, observing sterile technique if indicated.
(E)Assist physician with procedure.
(F)Instructs patient on appropriate after care.
13. Performs venipunctures and collects specimen samples for laboratory testing. Performs CUA wavered in house lab testing in accordance to Clinic policy and procedure. Orders, collects, stores, and sends laboratory testing to LabCorp in accordance to LabCorp's policy and procedure.
14. Schedules diagnostic testing or treatment to be done within the clinic or at another facility. Instructs patient on the prep required. Informs the patient of the date and time of the appointment and directions on where to go. Sends any pertinent orders or information to the receiving facility.
15. Obtains prior authorization from insurance companies when requested.
16. Collects and monitors diagnostic reports. Is responsible for knowing the normal values and alerting the physician of abnormal findings in a timely fashion. Provides reports with the patient's chart for physician review and continuing care orders.
17. Responsible for notifying patients and explaining findings (per permission of M.D.). Instruction patients on physician continuing care orders and follow-up plan.
18.Phone consultations- Liaison between, patient and physician. Assesses a situation over the phone. Documents it, conveys to physician, obtains instruction form physician, instructs patient and carries out orders appropriately.
19.Pharmacy call-ins- If a patient is not in the clinic when the doctor orders a medication the nurse will call the order to the patients preferred pharmacy, following policy and procedure protocol.
20.Documents all actions according to guidelines for quality documentation.
21.Provides patient education that enables the patient to make intelligent, inform.d decisions about their health and lifestyle.
22.Responsible for proper use and monitoring of supplies and equipment.
23.Completes other assignments form physician, Clinical Lead, or Clinic Director.
