Type of role: Part-Time Fee for Service (Minimum of 10 clinical hours/week)

Stokes Counseling Services is a leader in mental health private practice and group practice within the state of Connecticut. We are made up of licensed clinicians (LPC, LMFT, LCSW, LPCA, LMSW, MFTA) who provide an array of services, including individual, family, and group counseling. Officially headquartered in Naugatuck, CT, we also deliver telehealth services state-wide and in-home mental health therapy to specific areas. This position is based within our in-home WISE program, designed to service seniors, medically compromised and intellectually disabled communities, and/or their families/caregivers. We are a non-crisis outpatient level of care that allows clinicians to provide therapy services at a client's place of residence. Clinicians are not on call and are assigned to a set region of: Southington, CT. Personal transportation is required for this role.

What you’ll do:

Under the direction of a senior member of our Clinical Team, you will: 

  • Assess the mental and behavioral health status of referred clients with mild to moderate mental health conditions and/or substance abuse issues and Conduct standardized counseling/talk therapy sessions for Stokes Counseling clients
  • Perform intake/biopsychosocial assessments to evaluate clients' mental and behavioral health status.
  • Formulate differential diagnoses along with short-term and long-term goals. 
  • Provide timely and professional therapeutic services to scheduled clients utilizing professional industry standards.
  • Identify and refer clients with urgent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) to appropriate emergency or psychiatric treatment facilities.
  • Provide and maintain written documentation of intake, assessments, progress/contact notes, interventions, treatment plans, and termination notes within the designated time frame.
  • Maintain an understanding of how behavioral health priorities are administered under the State of Connecticut and consult with specialists and colleagues to review and participate in the care of clients.
  • Maintain appropriate documentation of clinical and counseling services, including all necessary billing information. Ensure accurate and consistent use of intake forms for all client contracts with behavioral health providers to ensure accurate billing of third-party resources. 
  • Charge copays, deductibles, co-insurance, and no-show/cancellation fees at the time of service when indicated and appropriate. 
  • Maintain and update professional knowledge and proficiency through continuing education, staff meetings, workshops, and serving on special committees. 

What we are looking for:

  • Fully licensed mental health therapist in the State of Connecticut (LPC, LMFT or LCSW).
  • One to three years of experience conducting counseling/talk therapy sessions with diverse client populations.
  • Advanced proficiency in conducting intake assessments and formulating treatment plans.
  • Strong written communication and interpersonal skills, including the ability to provide and maintain documentation within the appropriate time frames.
  • Ability to work collaboratively in a team-oriented environment.
  • Commitment to maintaining the highest ethical and professional standards in clinical practice.
  • Preferred previous experience working in the medical field, with seniors and/or intellectual disabilities clients. 
  • Passion for in-home work and working with the senior community. 
  • Have a valid driver's license and access to a properly insured vehicle for work or reliable transportation.

*You will receive a $1,000 sign-on bonus after completing six months of employment, provided you maintain the required 40 clinical hours per month.

WISE Clinician Fee for Service Rates:

  • Fully licensed clinicians:
    • $60 - intake, one-hour session, one-hour meeting, family sessions
    • $45 – for 45-minute sessions
    • $30 – for 30-minute sessions
