POSITION PURPOSE:

Under the direction of the department Supervisor and the Director of Clinical Operations, the Medical Records Clerk utilizes established procedures to prepare files, pulls and files medical charts, associates incoming medical reports with appropriate patient records, and files reports and documentation in patient medical records. Ability to keep several tasks moving along in a well-organized manner while functioning at a high level of accuracy is critical.

 

ESSENTIAL FUNCTIONS:

  • Pulls all charts for incoming established patients.
  • Pulls charts for patient correspondence and notification of test results.
  • Maintains efficient filing system, pulls and file patient charts, files outside reports and correspondence, and ensures all filing is completed at the end of each day.
  • Processes medical records requests within fifteen working days of receipt, following medical records release procedure. Logs information request in patient’s chart.
  • Maintains strict code of confidentiality, adhering to all HIPAA and privacy rules and regulations at all times.
  • Performs a wide range of general office procedures necessary to ensure the smooth operation of the Clinic.

  • Collaborates with immediate supervisor to establish, define, and refine department goals and objectives consistent with Aviva Health’s Mission, and demonstrates satisfactory progress towards these goals and objectives.
  • Exhibits the ability to make effective decisions based on verifiable and measurable data/criteria.

  • Demonstrates respect and sensitivity to cultural/social differences in interactions with others.
  • Completes assigned tasks in a timely and efficient manner.
  • Demonstrates initiative and problem-solving skills using sound judgement.

 

QUALIFICATIONS:

  • High school diploma or GED.
  • Ability to organize materials for filing in alphabetical and numerical order accurately. Must be able to work without supervision, be organized and self-motivated. Good typing skills required. Good communication and interpersonal skills.
