About Us: At Forward Service Corporation we're dedicated to empowering individuals to achieve self-sufficiency through our Foodshare Employment & Training (FSET) program. As a Case Manager, you'll be at the forefront of this mission, providing essential case management services and support to our program participants. This position is located at our office in Sheboygan, WI.

Key Responsibilities:

  • Serve as the primary case manager for the FSET program, assisting individuals in assessing their needs and reaching self-sufficiency goals.
  • Provide personalized employment counseling, job search assistance, and job placement services.
  • Develop and maintain strong relationships with participants, partners, and other stakeholders.
  • Conduct interviews and assessments to match participants with suitable employment opportunities.
  • Deliver engaging workshops and presentations on job readiness and retention techniques.
  • Ensure accurate and timely data entry into our systems for program tracking and reporting.
  • Provide referrals and assistance to connect participants with educational and supportive services.
  • Maintain confidentiality and uphold quality assurance standards in all aspects of case management.

Qualifications:

  • Associate degree and two or more years of relevant work experience, or equivalent combination of education and experience.
  • Knowledge of Foodshare Employment & Training and related programs preferred.
  • Strong ability to provide strength-based case management.
  • Proficiency in Microsoft Office software.
  • Excellent communication skills, both verbal and written.
  • Ability to work effectively in a fast-paced, team-oriented environment.
  • Valid driver's license with an acceptable driving record.
