Job Summary

Responsibilities
  • Perform intake assessments
  • Provide mental health therapy for individuals, couples, families, and children
  • Create individualized treatment plans for each client
  • Use evidence-based treatment methods to facilitate group and individual treatments
  • Maintain detailed and accurate documentation of patient information and treatment plan
  • Adhere to all facility and licensing standard
  • Conduct interventions that are consistent with the agency service model (values, beliefs, structure and practices) and with DCS Service Standards
  • Provide individual and /or family case management/therapeutic services to referred clients.
  • Assist in planning and/or facilitating other groups as necessary
  • Maintain a strength-based approach with all clients.
  • Schedule and complete client intake with face-to-face contact with the family within 48 hours of referral.
  • Provide ongoing risk assessment and monitoring family/parental progress
  • Provide services that include development of short and long-term family goals with measurable outcomes that are consistent with the DCS case plan. (SMART goals)
  • Provide services that are family centered and child focused. They are to include intensive in-home skill building and must include after-care linkage.
  • Services include providing monthly reports, progress notes, treatment plans, and all other requested supportive documentation.
Supervisory Functions - Documentation

Maintain a high standard of professional documentation in every aspect of job responsibilities.
Complete case contact notes, group notes, and other service documentation within 48 hours of client sessions.
Provide Home Base Case Managers with regular updates on clients participation and progress in services provided.
Monitor Overtime- Staff should be sending their supervisor an email by Wednesdays if they are noticing they will be in
overtime each pay week. The overtime request form must provide a written explanation of why/how the overtime could not be avoided.
The staff are responsible for filling out the overtime approval form and obtaining a signature from their supervisor.
Monitor ratio and billable face to face hours
Full time Staff should be at 30 billable Face to Face hours per week.
Part time staff should be 15-20 billable face to face hours per week
If staff are not pulling in all their expected hours, you will be expected to help guide them in achieving their expected billable face-to- face hours.



Qualifications
Masters degree in social work, psychology, marriage and family, or related human service field,
Valid Drivers License.
Ability to plan, develop, implement, manage, and monitor a complex, multi-faceted division of the agency.
Ability to organize, analyze, interpret, and report complex regulations, requirements, and other data, and recommend needed
corresponding program design.
Must be comfortable in working as a collaborative team environment be able to be on call and willing to cover in the event of
emergencies, delegate job duties to others and contribute to an environment of inclusion and safety for all staff.
